3D T1-VIBE序列在青年志愿者骶髂关节软骨成像中的初步应用
Application of 3D T1-VIBE sequence for the sacroiliac joint cartilage imaging in young healthy volunteers
【摘要】 目的 探讨三维容积内插体部扫描(3D T1-VIBE)序列显示骶髂关节(SIJ)软骨的可行性和应用价值。方法 分别采用T1WI快速自旋回波(T1WI-TSE)、T2WI反转恢复(T2WI-TIRM)、质子密度快速自旋回波(PDWI-TSE)、3D T1-VIBE序列对30例青年志愿者进行SIJ扫描,着重观察SIJ软骨,通过2名MRI诊断医师按照预先设定的评分标准进行双盲法评分,测量SIJ软骨的信号噪声比(SNR)及SIJ骶侧、髂侧软骨的厚度,比较不同成像序列显示青年志愿者SIJ软骨的能力。结果 常规T1WI-TSE序列较好显示30例青年志愿者共60个(侧)SIJ中16个SIJ的骶侧、髂侧软骨及二者间隙;T2WI-TIRM和PDWI-TSE序列分别显示其中的14、18个SIJ软骨及间隙;而3D T1-VIBE序列显示了58个骶侧、髂侧软骨及关节间隙,且关节软骨显示更为清晰,3D T1-VIBE序列显示SIJ软骨的能力高于其他序列(P<0.05)。T1-VIBE图像SNR低于其他3个序列,但差异没有统计学意义(P>0.05)。青年志愿者髂侧软骨厚度平均为(1.01±0.15) mm,骶侧(1.55±0.37) mm,髂侧面软骨较骶侧面薄(P<0.05)。结论 3D T1-VIBE序列可分辨出青年志愿者SIJ骶侧、髂侧软骨及二者间隙,较其他常规序列能更好地显示SIJ软骨。
【Abstract】 Objective To evaluate the feasibility and application value of MR 3D T1-weighted gradient-echo imaging for sacroiliac joint(SIJ) cartilage in healthy volunteers.Methods A total of 30 young healthy volunteers during the period were enrolled randomly and four MRI sequences,including T1 WI-TSE,T2 WI-TIRM,PDWI-TSE and T1-VIBE,were performed(Germany’s Siemens Verio3.0T superconducting MRI scanner).SIJ cartilage were observed emphatically and,different imaging sequences were compared.Two radiologists independently assessed the depiction of SIJ cartilage,as well as the detection and grading according to 5-point scale.The signal noise ratio(SNR) of SIJ cartilage for different sequence were measured,calculated and compared.In addition,the total thickness of SIJ cartilage,the cartilage of iliac and sacral sides were measured respectively.Results Among 30 young healthy volunteers(60 SIJ s),16 articular cartilage(sacral and iliac side) and joint space were identified individually on T1WI-TSE,and 14,18 articular cartilage and joint space on T2 WI-TIRM and PDWI-TSE respectively.However,58 articular cartilage and joint space were identified on 3D T1-VIBE sequence.3D T1-VIBE sequence showed the articular cartilage in SIJ clearer than other MRI sequences,which scores in showing SIJ cartilage were statisticly higher than that of other three sequences.The difference among T1WI-TSE,T2 WITIRM and PDWI-TSE had no statistical significance(P> 0.05).3D T1-VIBE sequence provided a lower SNR compared with other three sequences,however there was no significant difference(P> 0.05).The cartilage of sacrum(1.01 ±0.15) mm was thicker than that of ilium’s(1.55±0.37) mm,but there was no significant difference(P<0.05).Conclusion 3D T1-VIBE sequence can identify SIJ cartilage,including sacral and iliac side and the gap between them in young healthy volunteers.3D T1-VIBE is a valuable technique for examination of SIJ cartilage,which can provide more information for clinic.
