无症状人群肺裂结节的薄层CT表现
Thin-section CT features of fissure-attached nodules in asymptomatic patients
【摘要】 目的 分析无症状人群肺裂结节的薄层CT表现。方法 回顾性分析120例有肺裂结节的无症状人群的初检CT及复诊CT,观察肺裂结节的薄层CT表现,包括结节大小、形态、密度、边界及与肺裂的关系等。结果 120例无症状人群共检出肺裂结节167个,平均长径5.46 mm±2.276 mm(2~15 mm),其中104个(61.1%)结节长径≤5 mm。典型肺裂结节163个(97.6%),不典型肺裂结节4个(2.4%),在薄层CT上均表现为与肺裂相连的、密度均匀的、边界光滑的实性结节。164个(98.2%)结节呈三角形(88/167,52.7%)或椭圆形(76/167,45.5%)。126个(75.4%)结节位于斜裂。164个(98.2%)结节与肺裂夹角呈钝角。随访过程中4个(2.4%)结节增大,容积倍增时间为863~4 228 d,但未见明显恶性形态学征象。结论 无症状人群肺裂结节薄层CT表现具有良性结节特征,大多数呈直径≤5 mm、三角形或类圆形、边界光滑、密度均匀的实性结节。
【Abstract】 Objective To analyze the thin-section CT features of fissure-attached nodules in asymptomatic patients.Methods The baseline and follow-up CT images of 120 asymptomatic patients with fissure-attached nodules were retrospectively analyzed. The thin-section CT findings include size, shape, density, interface, and the relationship with pulmonary fissure.Results There were 167 fissure-attached nodules of the 120 patients. The mean length of nodules was 5.46 mm±2.276 mm(2-15 mm). 104(104/167, 61.1%) nodules were less than 5 mm in length.163(163/167, 97.6%) nodules were typical and 4(4/167, 2.4%) nodules were atypical. All these nodules manifested as homogenous, solid, smooth interface, and widely adjacent to the fissure on CT images. Among those nodules, 164(164/167,98.2%) nodules were triangular(88/167,52.7%)or oval(76/167,45.5%).126(126/167,75.4%) nodules were abutting the major fissure. The angle between nodule and fissure was obtuse in 164(164/167,98.2%)fissure-attached nodules. During follow-up, 4(4/167, 2.4%) nodules increased in size and the corresponding volume doubling time(VDT) range was 863-4 228 days. However, no obvious malignant signs were found. Conclusion The thin-section CT images of fissure-attached nodules in asymptomatic patients showed the benign characteristics, most of which less than 5 mm and demonstrated triangle or oval, smooth interface and homogenous solid density.
