肾透明细胞癌肾周侧支静脉的MDCT评价

【摘要】 目的 采用MDCT重建技术观察肾透明细胞癌肾周侧支静脉的发生情况。方法 29例确诊肾透明细胞癌病例均行三期增强MDCT扫描,记录肾周侧支静脉的发生情况和癌肿的影像学特征。结果 (1)肾透明细胞癌伴有肾周侧支静脉存在的发生率为64.4%;左侧发生率(81.8%)高于右侧(16.7%),差异有统计学意义(P<0.05)。(2)与对照组相比,伴有肾周侧支静脉的肾透明细胞癌癌肿体积较大(最大径8.42 cm±3.55 cm)、多伴有包膜受侵(96.6%)、静脉癌栓(44.8%)及动静脉瘘(34.5%)等征象,差异有统计学意义(P<0.05)。结论 肾透明细胞癌常伴有肾周侧支静脉的形成,癌栓引起的肾静脉回流受阻和癌肿动脉灌注增加是产生此征象的主要原因。

【Abstract】 Objective To observe the occurrence of renal clear cell carcinoma(RCCC) with perirenal collateral veins using multidetector computed tomography(MDCT) reconstruction technique.Methods Three phases enhanced MDCT scanning was performed in 45patients diagnosed with RCCC.The occurrence of perirenal collateral veins with RCCC and the imaging characteristics of the renal tumor were recorded.Results(1) The incidence of RCCC with perirenal collateral vein was 64.4%.The incidence of left kidney was81.8% higher than that of right kidney(16.7%),and the difference was statistically significant(P<0.05).(2) Compared with control group,RCCC with perirenal collateral veins have larger volume(maximum diameter of 8.42 ± 3.55 cm),and more capsular invasion(96.6%),more venous tumor thrombus(44.8 %) and more arteriovenous fistula(34.5%),the difference was statistically significant(P<0.05).Conclusion Perirenal collateral veins of RCCC are common.The obstructed renal venous flow caused by renal vein tumor thrombus and the increased artery perfusion of renal tumor are the main reasons of this sign.
