【摘要】 [目的]评价和比较爱丁堡产后抑郁量表(EPDS)3种分量表在高危妊娠孕妇心理筛查中的应用效果。[方法]采用方便抽样法,在产前检查门诊选择842例高危妊娠孕妇,进行EPDS测评,计算各分量表的敏感度、特异度、受试者工作特征曲线(ROC)下面积等以评价其筛查能力。[结果]高危妊娠孕妇EPDS总评分为(4.90±3.84)分;情感缺失分量表评分为(0.26±0.66)分;焦虑分量表评分为(3.33±2.31)分;抑郁分量表评分为(1.31±1.58分);焦虑分量表和抑郁分量表ROC曲线下面积均大于0.9,筛查能力较高;焦虑分量表以5分为临界值、抑郁分量表以3分为临界值时,敏感度、特异度、阳性预测值和阴性预测值指标间能达到较好的平衡;EPDS 3个分量表分值与其总评分均呈正相关(P<0.01)。[结论]高危妊娠孕妇的心理问题不容忽视,EPDS分量表具有快速、简便而高效的心理筛查效果,有助于尽早发现和干预产后抑郁的高危人群。更多还原

【Abstract】 Objective:To evaluate and to compare the application effect of the three subscales of EPDS for psychological screening of high risk pregnant women.Methods:A total of 842 high risk pregnant women from prenatal clinics were selected to receive the EPDS assessment by the convenient sampling method.Then to calculate the sensitivity,specificity,the area under receiver operating characteristic curve(ROC)of each subscale,so as to evaluate their screening capacity.Results:The total score of EPDS of high risk pregnant women was 4.90±3.84.The lack of emotion subscale score was 0.26±0.66.The anxiety subscale score was 3.33±2.31.The depression subscale score was 1.31±1.58.The area under ROC curves of the anxiety subscale and the depression scale were all bigger than 0.9.Showing the better screening effects.When the anxiety subscale considered 5as the critical value,the depression scale considered 3as the critical value,the sensitivity,specificity,positive predictive value and negative value index could achieve a better balance.The scores of three subscale of EPDS had the positive correlation with its total score(P<0.01).Conclusion:The psychological problems of high risk pregnant women can not be ignored.The EPDS subscales has fast,simple and efficient psychological screening effect.It is helpful to discover and to intervene the high risk group of postpartum depression as early as possible.更多还原
