【摘要】 目的探讨应用扩张的上臂皮瓣行烧伤后全耳郭再造的方法 及相关问题。方法 手术分为6期:Ⅰ期上臂内侧置入皮肤软组织扩张器,完成注水扩张过程;Ⅱ期取出上臂内侧扩张器,形成皮管,同时行头部扩张器置入;Ⅲ期皮管转移至耳郭位置;Ⅳ期断蒂,同时耳后置入扩张器,完成扩张;Ⅴ期取肋软骨行全耳再造;Ⅵ期取出头部扩张器,切除头部瘢痕。结果 术后9个月,再造的耳郭外形轮廓良好,立体感强,耳郭无明显移位或吸收,患者满意。结论 该方法 再造的耳郭外形良好。对于烧伤后头颈部遗留明显瘢痕的患者,是切实可行的手术方法 。更多还原

【Abstract】 Objective To investigate the operating methods and its relating questions of total auricular reconstruction with expanded upper arm flap transfer in the repairing of auricular defects after bum.Methods The procedure were divided into the 6 stages:(1) implantation of tissue expander at the inner upper arm and affusion;(2) removal of the expander to form skin tube and placement of scalp expander at the same time;(3) transfer of skin tube to the auricle;(4) cutting of skin tube pedicle and implantation of retroauricular expander;(5) reconstruction of whole auricular with rib cartilage framework transfer;(6) removal of scalp expander and excision of scalp scar.Results All patients were satisfied with the reconstructed auricle with good sense of three-dimensional contour and without misplacement and absorption at 9 months postoperatively.Conclusion Auricular reconstruction with expanded upper arm flap transfer and consequent expansion is a practicable method for the patients with obvious scar on head and neck after burn.更多还原
