【摘要】 目的探讨利用改良原甲板回植法治疗甲床损伤的临床疗效。方法选取2013年6月至2014年6月间入院甲床损伤患者共68例74指,男42例,女26例;年龄11~68岁,平均40.2岁。其中改良组(34例37指)采用剪除部分原甲板远端后原位贴附、简单固定的改良回植方法治疗;传统组(34例37指)采用修复甲床后油纱覆盖、包扎传统方法治疗。评价两组治疗并发症发生率,新生甲外形采用吕桂欣疗效评价标准评定优良率。结果改良组发生甲床坏死或创面感染并发症发生率明显低于传统组,差异有统计学意义。两组患者均得到术后随访3~5个月,采用吕桂欣疗效评价标准对新生甲进行评定:改良组37指中优良35指,差2指,优良率94.6%;传统组37指中,优良30指,差7指,优良率81.1%。改良组新生甲外形优良率明显高于传统组,差异有统计学意义。结论与传统方法相比,改良原甲板回植法是修复甲床损伤更为有效的方法,操作简单,创伤小,并发症少,新生甲外形更优良。更多还原

【Abstract】 Objective To discuss the therapeutic effect of the treatment of injuried nail bed with modified original nail plate replantation.Methods From June 2013 to June 2014,74 fingers nail bed injuries in 68 cases were involved in the study,including 42 males and 26 females.Among them,the modified group including 37 fingers in 34 cases underwent the modified we replantation method of the distal original nail plate partal pruned and we reattched in situ with simple fixation to repair.The traditional group including 37 fingers in 34 cases treated with the traditional method underwent the vaseline dauzes to dress the sutured nail beds.The incidence of complications and the good or excellent rate of new nails evaluated according to the assessment criteria of Lv gui-xin were compared between the two groups.Results The incidence of nail bed necrosis and infection complications in the modified group was significantly lower than that in the traditional group,the difference was statistically significant.All the cases in two groups were follow-up for 3 to 5 months,the new nails were evaluated according to the assessment criteria formulated by Lv gui-xin: the results of 37 fingers in the modified group were good or excellent in 35 fingers,fair in 2fingers,the good or excellent rate was 94.6%; the results of 37 fingers in the traditional group were good or excellent in 30 fingers,fair in 7 fingers,the good or excellent rate was 81.1%.The good or excellent rate of new nail bed in the modified group was significantly higher than that in the traditional group,the difference was statistically significant.Conclusion The use of modified original nail plate replantation is a more effective method for repair nail bed injuries.This approach is easier,less traumatic and causes fewer complications than the traditional method and can get more satisfactory new nails.更多还原
