丙戊酸钠治疗BECTS异常放电的应用价值

【摘要】 目的探讨丙戊酸钠治疗儿童良性癫痫伴中央-颞区棘波(BECTS)异常放电的有效血药浓度及对患儿认知功能的影响。方法选取2012年1月至2013年12月期间收治的30例BECTS患儿(观察组)和30例非BECTS癫痫患儿(对照组),均给予丙戊酸钠小阶梯逐渐加量至异常放电消失,观察两组有效血药浓度及观察组相应状态下患儿认知功能的变化。结果观察组患儿异常放电得到控制后丙戊酸钠血药浓度平均值为(28.76±7.04)μg/mL,而对照组患儿异常放电得到控制后丙戊酸钠血药浓度平均值为(70.43±5.87)μg/mL,两组血药浓度比较具有显著性差异(t=8.98,P<0.05);观察组患儿异常放电控制后其总智商(FIQ)较治疗前明显增加(t=5.78,P<0.05);观察组患儿异常放电控制后其持续注意力操作测试(CPT)正确数明显增加而错误数明显减少(t值分别为6.43、7.02,均P<0.05)。结论控制BECTS癫痫患儿异常放电的丙戊酸有效浓度明显低于其他类型的癫痫,且在此血药浓度下患儿注意力和认知功能均得到明显改善。

【Abstract】 Objective To explore the effective blood concentration of sodium valproate in treatment of abnormal discharge in benign epilepsy with centrotemporal spikes(BECTS) and the influence on cognitive function of patient children.Methods From January 2012 to December 2013 30 cases of BECTS(observation group) and 30 cases of non-BECTS(control group) were enrolled,and sodium valproate was given to them with gradually increased dosage until the epileptic discharge disappeared.The effective blood concentration of two groups and the changes of cognitive function in observation group were observed.Results The average blood concentration of sodium valproate after controlling of abnormal discharge in the observation group was 28.76 ±7.04μg/mL,which was significantly lower than that in the control group(70.43 ± 5.87 μg/mL)(t=8.98,P<0.05).The full-scale intelligence quotient(FIQ) after the controlling of abnormal discharge in the observation group was significantly increased compared with that before treatment(t=5.78,P<0.05).In CPT the correct number increased and the error number decreased significantly after the controlling of abnormal discharge in the observation group(t value was6.43 and 7.02,respectively,both P<0.05).Conclusion The effective blood concentration of sodium valproate in controlling abnormal discharge of BECTS is significantly lower than that of other types of epilepsy.Patient children’ s attention and their cognitive ability are significantly improved with this concentration.
