【摘要】 目的探讨α-1-微球蛋白/胰蛋白酶抑制剂前体(alpha-1-microglobulin/bikunin precursor,AMBP)在胃癌患者血清中的表达及其在胃癌早期诊断的价值。方法收集北京大学肿瘤医院2007-07-05-2008-12-20收治的114例胃癌患者和49名健康体检者的血清,114例胃癌患者分为胃癌早期组(49例)和胃癌晚期组(65例)。采用蛋白质印迹法和酶联免疫吸附试验(enzyme-linked immunosorbent assay,ELISA)检测血清中AMBP水平,分析其与胃癌临床病理参数的相关性。结果蛋白质印迹法检测结果显示,正常组AMBP血清平均表达量为22.62±2.91,胃癌早期组为32.19±1.53,胃癌晚期组为41.61±3.19,差异有统计学意义,F=12.04,P=0.002 2;组间两两比较差异亦有统计学意义,P值均<0.05。ELISA检测结果显示,正常组AMBP血清平均表达量为(8.156±0.154)μg/mL,低于胃癌患者的(8.948±0.107)μg/mL,t=4.140,P<0.001;在95%可信度下,取阈值为8.546μg/mL时,其诊断敏感性为71.05%,特异性为63.27%;胃癌早期组AMBP血清平均表达量为(8.717±0.173)μg/mL,晚期组为(9.122±0.131)μg/mL,均高于正常组(8.156±0.154)μg/mL,P值均<0.05。AMBP血清表达量与患者年龄(P=0.153 9)、性别(P=0.143 1)、肿瘤大小(P=0.080 9)、淋巴结转移(P=0.073 7)无关,而与肿瘤侵袭程度有相关性,P=0.021 8。结论胃癌患者血清AMBP水平显著升高,可能是胃癌早期诊断潜在肿瘤标志之一。更多还原

【Abstract】 OBJECTIVE To explore the expression of AMBP(α-1-Microglobulin/Bi-kunin precursor)in the serum of patients with gastric cancer and the clinical value of early diagnosis for gastric cancer.METHODS 114 patients with gastric cancer and49 healthy controls were selected as research objects from 2007-07-05 to 2008-12-20,Peking University Cancer Hospital.114 cases of gastric cancer patients were divided into early gastric carcinoma group(49cases)and late gastric cancer group(65cases).Serum levels of AMBP were determined by Western blot and Enzyme-linked immunosorbent assay(ELISA).GraphPad Prism 5.01 and T-test were used for analyzing the clinical value of AMBP in early diagnosis and judging the clinical biological behavior of gastric cancer.RESULTS The westem blotting result showed the mean expression of AMBP was 22.62±2.91,which were 32.19±1.53and41.61±3.19 in gastric cancer group and control group,respectively(F=12.04,P=0.002 2).The contents of AMBP in the serum of gastric cancer group was(8.948±0.107)μg/mL,which was obviously higher than that of control group(8.156±0.154)μg/mL,t=4.140,P<0.001.When 8.546μg/mL was chosen as a cut-off,the sensitivity and specificity for AMBP in the diagnosis of gastric cancer were 71.05%and 63.27%.The mean serum expression levels of AMBP in healthy controls,early gastric cancer group and late gastric cancer group respectively were(8.156±0.154),(8.717±0.173)and(9.122±0.131)μg/mL.Serum level of AMBP was significantly higher in early gastric cancer group and late gastric cancer group than that of health control group(P<0.05;P<0.001).There was no correlation between serum AMBP and age(P=0.153 9),gender(P=0.143 1),tumor size(P=0.080 9)and lymphatic metastasis(P=0.073 7),but was closely associated with depth of invasion(P=0.021 8).CONCLUSION Serum AMBP levels are significantly elevated in gastric cancer patients,it may be used as a potential biomarker in early diagnosis of gastric cancer.更多还原
