广西壮族类风湿关节炎患者HLA-DR基因表达及其与临床指标的相关性研究
HLA-DR Gene Expression of Zhuang Ethnic Patients With Rheumatoid Arthritis and Its Correlation With Clinical Indicators
【摘要】 目的了解广西壮族类风湿关节炎(RA)患者HLA-DR基因的表达情况,探讨RA患者HLA-DR基因的表达与抗环瓜氨酸肽(CCP)抗体、类风湿因子(RF)、C反应蛋白(CRP)的相关性。方法选取2008年1月—2013年2月右江民族医学院附属医院风湿科收治的广西壮族RA患者49例为RA组,另选取同时期本院体检健康的广西壮族成年人54例为对照组,采用序列特异性引物-聚合酶链式反应(SSP-PCR)法检测HLA-DR基因表达,双抗体夹心酶联免疫吸附试验(ELISA)法检测抗CCP抗体,免疫比浊法检测RF、CRP水平。结果 RA组抗CCP抗体阳性率、RF、CRP水平高于对照组(P<0.05)。两组均可检测出DRB1*03、DRB1*04、DRB1*08~DRB1*17、DRB3*01、DRB4*01、DRB5*01亚型;RA组DRB1*10、DRB4*01阳性表达率均高于对照组(P<0.05);两组其余13个亚型阳性表达率比较,差异均无统计学意义(P>0.05)。DRB1*10阳性RA患者与DRB1*10阴性RA患者、DRB3*01阳性RA患者与DRB3*01阴性RA患者、DRB4*01阳性RA患者与DRB4*01阴性RA患者、DRB5*01阳性RA患者与DRB5*01阴性RA患者抗CCP抗体阳性率、RF、CRP水平比较,差异均无统计学意义(P>0.05)。结论广西壮族RA患者HLA-DR基因DRB1*10、DRB4*01亚型可能为易患基因,但DRB1*10、DRB3*01、DRB4*01、DRB5*01的阳性表达均与抗CCP抗体、RF、CRP无相关性。
【Abstract】 Objective To explore the HLA- DR gene expression of Zhuang ethnic patients with rheumatoid arthritis( RA) and investigate its correlation with anti- cyclic citrullinated peptide( CCP) antibody,rheumatoid factor( RF) and C-reactive protein( CRP). Methods 49 Zhuang ethnic patients with RA who received treatment in the Department of Rheumatism of the Affiliated Hospital of Youjiang Medical University for Nationalities from January 2008 to February 2013 as the RA group. And another 54 Zhuang ethnic healthy adults who received physical examination in the same hospital during the same period were enrolled as the control group. PCR- SSP method was used to detect HLA- DR gene expression,ELISA was employed to detect anti- CCP antibody,and immunoturbidimetry was used to detect the levels of RF and CRP. Results The RA group was higher in positive rate of anti- CCP antibody and the levels of RF and CRF than the control group( P < 0. 05). Noted DRB1*03,DRB1* 04,DRB1* 08- 17,DRB3 * 01,DRB4 * 01 and DRB5 * 01 in both groups,and RA group was higher( P <0. 05) than control group in the positive expression rates of DRB1 * 10 and DRB4 * 01; the two groups were not significantly different( P > 0. 05) in the positive expression rates of the rest 13 gene subtypes. No significant differences( P > 0. 05) were noted in the positive rate of anti- CCP antibody and the levels of RF and CRF between DRB1* 10 positive group and DRB1* 10 negative group,between DRB3* 01 positive group and DRB3* 01 negative group,between DRB4* 01 positive group and DRB4* 01 negative group and between DRB5* 01 positive group and DRB5* 01 negative group. Conclusion DRB1 * 10 and DRB4* 01 may be the susceptible genes in Zhuang ethnic patients with RA,while the positive expressions of DRB1* 10,DRB3* 01,DRB4* 01 and DRB5* 01 have no correlation with anti- CCP antibody,RF and CRP.
