空心取骨环钻取骨原位植入并上颌窦内提同期种植的临床初步评价
Preliminary clinical evaluation of the hollow bone ring drill bone in situ implantation and donetti maxillary implant
【摘要】 目的对牙槽骨高度较低时用空心取骨环钻取骨原位植入并行上颌窦内提升同期植入种植体的疗效进行初步评价。方法对上颌后牙区剩余牙槽骨高度不足(特别5 mm及以下)的15例患者,应用空心取骨环钻取骨后原位植入并上颌窦内提同期种植,共植入种植体21颗,术后随访观察半年到1年半,通过临床观察及影像学检查的方法进行评估。结果 15例患者中,术前上颌窦底至牙槽嵴顶的距离为3~5 mm,术中提升上颌窦底高度为4~7 mm,同期植入种植体,无一例患者发生上颌窦黏膜穿孔,患者随访期内,均未出现上颌窦炎等并发症,种植体无松动、脱落,成功率达100%。结论应用空心取骨环钻取骨后原位植入行上颌窦内提升同期种植,特别是牙槽骨高度较低(5 mm及以下)时,具有手术创伤小、操作安全简便、基层医院易于开展等特点,其短期疗效较好,长期疗效有待于进一步观察。
【Abstract】 Objective To investigate and evaluate the clinical effect of the Hollow Bone Trephine applied to maxillary sinus fl oor elevation with simultaneous placement of implants. Methods Fifteen patients underwent maxillary sinus fl oor elevation with simultaneous implant placement from the top of alveolar ridge using the Hollow Bone Trephine. All the patients had severely absorbed alveolar bone. Twety-one implants were inserted and restored. The follow-up period after operation was 6 to 18 months. Clinical examination and radiography were conducted. Results The residual bone height was 3 to5 mm, in 15 patients. The elevation height ranged from 4 mm to 7mm,Implants were placed simultaneously. There was no detectable sinus membrane perforation, no suffering or uncomfortable subjective sensation follow in any patients during operation. No sinus complication was observed during the follow-up time. There were no implants or prostheses loose or lost. The survival rate was 100%. Conclusions Maxillary sinus fl oor elevation with simultaneous implant placement by Hollow Bone Trephine is a safe, invasive and handy technique. It shows satisfactory clinical results in short term and a long-term observation is still needed.
