2012—2014年某院女性生殖道支原体感染及耐药性变迁分析
Detection and variation of drug resistance of mycoplasma causing female genital tract infection
【摘要】 目的探讨女性生殖道支原体感染及耐药性变化趋势,为指导临床合理用药提供依据。方法收集笔者所在医院就诊患者共4607例女性生殖道分泌物标本进行支原体培养,并对支原体阳性培养结果和体外药敏试验结果进行分析。结果女性生殖道支原体感染以解脲脲原体(Uu)单一感染为主,占72.7%,Uu+Mh(人支原体)混合感染占20.5%,Mh单一感染占6.8%。药敏试验结果显示,Uu+Mh混合感染和Mh单一感染的耐药性较为严重,对除了交沙霉素外的其他大环内酯类抗菌药物的耐药率均>85%。对近3年的检测结果进行分析,Uu单一感染对环丙沙星和克拉霉素的耐药率逐年增高,而Uu+Mh混合感染对左氧氟沙星的耐药率也有逐年增高的趋势。结论女性生殖道支原体感染阳性检出率较高,且对部分抗菌药物有较高的耐药性,应根据患者各自的药敏实验结果进行针对性用药,以提升治疗效果且避免耐药菌株的出现。
【Abstract】 Objective To investigate the incidence of female genital tract infection caused by mycoplasma,analyze the trend of drug resistance,and provide evidence for reasonable use of antibiotics. Methods A total of4607 female genital tract specimens were collected from the patients with potential genital tract infection,which were cultured for detecting mycoplasma. The results of the specimens cultured positive and drug susceptibility testing were analyzed. Results Among all the cases of mycoplasma female genital tract infections,the cases of single Uu infection were dominant,accounting for 72.7%,the cases of mixed Uu and Mh infection accounting for20.5%,and the cases of single Mh infection accounting for 6.8%. Susceptibility testing showed that the drug resistance of Uu +Mh and single Mh infection were more severe,which resistance rate to macrolide antimicrobial drug were all more than 85% except for josamycin. In the results of mycoplasma testing in nearly 3 years,the resistance rate to ciprofloxacin and clarithromycin was also increasing year by year in Uu single infection,and the same was the levofloxacin resistance rate of Uu+Mh. Conclusion The drug resistance of mycoplasma is severe in female genital tract infection. Drug susceptibility test is necessary for using antibiotics reasonably,enhancing the therapeutic effect and avoiding the emergence of resistant strains.
