动态心电图的心电轴分析

【摘要】 目的探讨动态心电图中心电轴变化规律及影响因素。方法入选窦性额面QRS电轴偏移的患者313例,根据电轴偏移情况分为三组,其中电轴左偏组205例,电轴右偏组97例,无人区电轴组11例。观察电轴偏移程度、动态变化情况及与心率的关系。结果三组电轴偏移受试者的性别构成比无统计学差异(P>0.05);电轴左偏组及无人区电轴组受试者年龄均显著大于电轴右偏组(P<0.01);无人区电轴组受试者合并疾病的比例显著高于电轴左偏组(P<0.05)和电轴右偏组者(P<0.01);电轴左偏组合并疾病的比例显著高于电轴右偏者(P<0.01);三组受试者,均有较大比例电轴不固定而呈动态变化,组间比较存在统计学差异(P<0.01);电轴的动态变化多数受试者与心率相关,且随心率增快电轴偏移程度增加,组间比较有统计学差异(P<0.05)。结论电轴偏移较大比例呈动态变化及频率依赖性。随年龄增长电轴向左和/或无人区偏移,随心率增快电轴偏移程度增加。无人区电轴最具病理意义。

【Abstract】 Objective To discuss the change laws and influencing factors of cardiac electric axis in dynamic electrocardiogram(DCG). Methods The patients(n=313) with sinus frontal QRS axis offset were chosen and divided into 3 groups according to axis offset status: group 1(with left axis deviation, n=205), group 2(with right axis deviation, n=97) and group 3(with axis of no man’s land, n=11). The degree and dynamic changes of axis offset, and correlation between these changes and heart rate(HR) were observed. Results The sex constituent ratio has no statistical difference in 3 groups(P>0.05), and age was significantly higher in group 1 and group 3 than that in group 2(P<0.01). The percentage of patients with complications was significantly higher in group 3 than that in group 1(P<0.05) and group 2(P<0.01), and that was significantly higher in group 1 than that in group 2(P<0.01). In 3 groups, a large percentage of patients had dynamic axis changes due to offset, and there was statistical difference in comparison among 3 groups(P<0.01). The dynamic axis changes was correlated to HR, and exacerbated as HR increased, and there was statistical difference in comparison among 3 groups(P<0.05). Conclusion The axis offset is correlated to aging, and a large percentage of patients has dynamic axis changes and frequency dependence. As age increases, axis will deviate to the left and/or no man’s land, and deviation degree will exacerbate as HR increases. The axis of no man’s land has the highest pathological significance.
