【摘要】 目的 探讨前列腺包膜在大体积前列腺增生（>100g）TURP手术中的作用。方法 回顾性分析我院2010年6月至2013年12月期间,60例行TURP手术的超大腺体前列腺增生（>100g）患者的临床资料,比较术前术后患者的排尿情况,术后包膜穿孔者与未穿孔者血糖、血钠变化及TURS的发生率。结果 60例患者术后排尿情况明显改善,术后IPSS、QoL、Q_max较术前均有显著差异（P<0.05）。8例术中出现不同程度包膜损伤或穿孔,7例术中出现明显血糖升高及血钠降低,考虑TURS发生,其中6例合并包膜损伤。结论 前列腺包膜在超大前列腺TURP手术中有重要意义,术中保持包膜完整性是手术成功的关键,能显著降低术中TURS发生率。更多还原

【Abstract】 Objective To investigate the value of prostate capsule on transurethral resection of the prostate with large volume.Methods A retrospective case analysis was performed in this study.Clinical data of 60 patients with large volume benign prostatic hyperplasia who underwent TURP in our hospital from June 2010 to December 2013 were comparatively analyzed.The recovery of urination after operation was observed.All the patients were divided into capsule perforation group and non capsular perforation group.Levels of blood sugar,blood sodium and the incidence rate of TURS in these two groups were comparatively analyzed.Results Urination after operation was improved for all patients.The mean IPSS symptom score and Q_max were improved significantly,whereas QoL index were decreased（P<0.05）.Eight patients had prostate capsule perforation during operation,and 7 patient of them had high level of blood glucose and low level of blood sodium,6 cases combined prostate capsule perforation.Conclusion Prostatic capsule has the vital significance in the transurethral resection of the prostate with large volume,and keeping prostatic capsule integrity is the key for success in surgery,it can significantly reduce the incidence of postoperative complications.更多还原
