2型糖尿病患者血尿酸与血浆致动脉粥样硬化指数的相关性研究
Association between serum uric acid level and atherogenic index of plasma in T2DM patients
【摘要】 目的探讨2型糖尿病患者血尿酸与血浆致动脉粥样硬化指数(AIP)的关系。方法选择1325例2型糖尿病患者,根据血尿酸水平分为尿酸正常组1020例和高尿酸组305例,采用酶法测定血尿酸水平。患者另按AIP四分位数分为A组330例、B组334例、C组327例、D组334例。结果A、B、C、D组高尿酸血症患病率分别为11.82%、23.35%、23.85%和32.93%(P=0.000)。高尿酸组AIP明显高于尿酸正常组(0.66±0.85 vs 0.37±0.82,P<0.01)。血尿酸水平与体脂(FAT)百分比、体质量指数(BMI)、TC、TG、载脂蛋白(apo)B、apoB/apoA、动脉硬化指数、血脂综合指数、AIP、非HDL-C、非HDL-C/HDL-C正相关,与HDL-C、估算的肾小球滤过率(eGFR)和基于胱抑素C的肾小球滤过率(Cys-C-eGFR)负相关(P<0.05,P<0.01)。校正性别、年龄、BMI和FAT百分比后,血尿酸水平与AIP正相关(P<0.01)。AIP、HDL-C、BMI、eGFR和Cys-C-eGFR是影响患者血尿酸水平的独立相关因素。logistic回归分析显示,AIP是预测2型糖尿病患者高尿酸血症发病的独立危险因素。结论 2型糖尿病患者血尿酸水平与AIP密切相关,可引起脂代谢紊乱和肾脏损害,参与动脉粥样硬化的发生、发展。
【Abstract】 Objective To study the association between serum uric acid(SUA)level and atherogenic index of plasma(AIP)in T2 DM patients.Methods A total of 1325T2 DM patients were divided into normal SUA group(n=1020)and high SUA group(n=305).Their SUA levels were measured with enzymatic method.The patients were further divided into group A(n=330),group B(n=334),group C(n=327)and group D(n=334)according to their quartiles of AIP.ResultsThe prevalence of hyperuricemia was 11.82%,23.35%,23.85% and 32.93%in groups A,B,C and D,respectively(P=0.000).The AIP was significantly higher in high SUA group than in normal SUA group(0.66±0.85 vs 0.37±0.82,P<0.01).The SUA levels were positively associated with FAT%,BMI,TC,apoB,apoB/apoA,arterial stiffness index,lipid comprehensive index,AIP,non HDL-C,and non HDL-C/HDL-C and negatively associated with HDL-C,eGFR and CysC-eGFR(P<0.05,P<0.01).After adjustment for gender,age,BMI and FAT%,the SUA levels were still poisitively associated with AIP in patients(P<0.01).AIP,HDL-C,eGFR and Cys-C-eGFR were the independent factors for SUA levels in patients.Logistic regression analysis showed that AIP was an independent risk factor for hyperuricemia in patients.Conclusion SUA level is closely related with AIP and can induce disorders of lipid metabolism and renal impairment,thus playing a role in the occurrence and progress of atherosclerosis in T2 DM patients.
