急性冠脉综合征患者细胞因子检测及临床意义

【摘要】 目的 探讨急性冠脉综合症患者冠脉病变程度与血清IL-1、IL-2、IL-4、IL-6、IL-8、TNF-α和IL-10水平变化关系。方法 选取2013年1月至2014年6月急性冠脉综合征患者120例作为观察组,同期选取健康献血者40例作为对照组,采用双抗体夹心法酶联免疫吸附试验(ELISA)测定白细胞介素-1(IL-1)、IL-2、IL-4、IL-6、IL-8、肿瘤坏死因子-α(TNF-α)和IL-10,分析观察组和对照组差异,观察组治疗前后差异及相关性分析。结果 急性冠脉综合征患者IL-1、IL-2、IL-4、IL-6、IL-8、TNF-α高于正常健康人,IL-10低于正常健康人,差异均有统计学意义(P<0.01);单支、双支及多支病变三个亚组血清细胞因子水平比较,差异有统计学意义(P<0.01),经两两比较,多支病变组血清促炎细胞因子水平较高,抑炎因子水平较低(P<0.01);冠状动脉介入(PCI)治疗后患者IL-1、IL-2、IL-4、IL-6、IL-8、TNF-α均明显降低,IL-10明显升高(P<0.01);冠脉病变支数和促炎细胞因子IL-1、IL-2、IL-4、IL-6、IL-8、TNF-α成正相关,与抑炎细胞因子IL-10成负相关。结论 急性冠脉综合征惠者冠脉病变程度与细胞因子水平关系密切,监测细胞因子水平可作为判断急性冠脉综合征患者冠脉病变程度及预后的指标。

【Abstract】 Objective To explore the relationship between the severity of coronary lesions in patients with acute coronary syndrome and changes of serum IL-1,IL-2,IL-4,IL-6,IL-8,TNF-and IL-10 levels.Methods From January in 2013 to June in 2014,120 cases with Acute coronary syndrome accepted treatment in our hospital was as the observation group,at the same period,40 cases normal person was as the control group,using double antibody sandwich enzyme linked immunosorbent assay(ELISA) for determination of IL-1,IL-2,IL-4,IL-6,IL-8,TNF-alpha and IL-10,the analysis of the observation group and the control group differences,to observe and analyze the difference and correlation between groups before and after treatment.Results The patients with acute coronary syndrome IL-1,IL-2,IL-4,IL-6,IL-8,TNF-alpha was higher than that in normal healthy people,IL-10 lower than normal healthy people,the difference was statistically significant(P<0.01);comparison of single branch,double branch and multi branch lesion in three subgroups of serum cytokine levels,the difference was statistically significant(P<0.01),The two two comparison,multi vessel disease group serum proinflammatory cytokines levels of inflammatory cells is higher,anti inflammatory factor level is low(P<0.01);after treatment,EL-1,IL-2,IL-4,IL-6,IL-8,TNF-alpha were significantly decreased,IL-10 increased significantly(P<0.01);coronary disease and proinflammatory cytokines IL-1,IL-2,IL-4,IL-6,IL-8,TNF-alpha is positive correlation,and anti inflammatory cytokines IL-10 negatively correlated.Conclusion Acute coronary syndrome in patients with coronary artery disease and degree of cytokine levels closely,factor level monitoring cell can be used to judge the degree of coronary artery lesions in patients with acute coronary syndrome and prognosis of patients with index.
