正常血压高值者颈动脉内膜中层厚度与肥胖指标的相关性
Relationship between Carotid Intima-media Thickness( IMT) and the Different Obesity Indices in Patients with High Normal Blood Pressure
【摘要】 目的探讨正常血压高值者颈动脉内膜中层厚度(IMT)与肥胖指标的关系。方法选择2013年1月—2014年11月健康体检者中正常血压高值者330例,根据颈动脉IMT值分为A组(IMT<0.6 mm)103例、B组(IMT 0.6~0.8 mm)177例及C组(IMT≥0.8 mm)50例;选择同期体检血压正常者170例作为对照组。均测定身高、体重、腰围(WC)、体重指数(BMI)、体脂量(BFM)、净体重(LW)及脂肪重量(FW)等肥胖指标,统计分析颈动脉IMT与不同肥胖指标间的相关性。结果 C组IMT及BMI、WC、BFM、FW均显著高于A、B组,组间比较差异均有统计学意义(P均<0.05);控制混杂因素后,颈动脉IMT与BMI、BFM、WC、LW、FW及收缩压(SBP)存在正相关性(P均<0.05),与舒张压不存在相关性(P>0.05)。以颈动脉IMT为因变量,BMI、BFM、WC、LW、FW及SBP为自变量进行逐步线性回归分析,结果显示BMI、WC、FW进入了回归方程(P均<0.05)。结论正常血压高值者SBP与颈动脉IMT呈正相关,BMI、WC及FW与颈动脉IMT的关系更为密切,可促进颈动脉粥样硬化的发展。
【Abstract】 Objective To investigate and analyze the relationship between carotid intima-media thickness( IMT) and the different obesity indices in patients with high normal blood pressure. Methods 330 cases of moderate normal blood pressave undergoing phgsical examinations during Jan. 2013 and Nov. 2014 in our hospital were selected and divided into group A according to IMT values( IMT < 0. 6 mm,n = 103),group B( IMT 0. 6- 0. 8 mm,n = 177); and group C( IMT≥0. 8 mm,n = 50). At the same,170 cases of normal blood pressure were selected as control group and all the cases were measared for height weight,waist circumference( WC),body mass index( BMI),body fat mass( BFM),lean weight( LW) and Fat Weight( FW). Results IMT in group C was significantly higher than those in group A and B; BMI,WC,BFM,FW in group C were significantly higher than those in group A and B,with statistically significant difference( P < 0. 05); After confounding factors were controlled,there existed a positive correlation between IMT and BMI,BFM,WC,LW,FW and SBP( P all < 0. 05). There was no correlation between IMT and DBP( P > 0. 05). As IMT was a dependent variable,as all the above correlation with IMT of BMI,BFM,WC,LW,FW and SBP as independent variable underwent stepwise linear regression analysis,the results showed that BMI,WC,FW entered the regression equation( P all < 0. 05). Conclusion There is a positive correlation between systolic blood pressure and IMT in NHBP group. There is a closer relationship between obesity index and IMT,which may attribute to IMT to carotid intimal thickening and carotid atherosclerosis development.
