【摘要】 目的探讨胎儿纤维连接蛋白(fFN)及宫颈长度(CL)变化对既往有早产史的孕妇进行早产预测的价值。方法对50例既往有早产史的单胎妊娠孕妇的宫颈分泌物fFN进行免疫检测,腹部超声检查测量CL,观察fFN+和fFN-、CL≤25mm和>25mm孕妇35周前发生自发性早产的情况。结果 50例孕妇中35周前发生自发性早产23例(46.0%,23/50)。fFN+孕妇(n=27)早产发生率显著高于fFN-孕妇(n=23)(P<0.001),fFN+与孕妇发生早产的风险有很高的关联[相对危险度(OR)为19.05,95%CI为4.30~84.33,P<0.05]。CL≤25mm孕妇(n=29)早产发生率显著高于CL>25mm孕妇(n=21)(P<0.001),CL≤25mm孕妇再次发生早产的风险很大(OR为8.08,95%CI为2.13~30.58,P<0.05)。CL≤25mm组中fFN+孕妇早产发生率显著高于fFN-孕妇(P<0.001);而CL>25mm组中fFN+和fFN-孕妇早产发生率比较差异无统计学意义(P>0.05)。结论 fFN结合CL测定对曾有自发性早产史的孕妇有一定的早产预测价值。更多还原

【Abstract】 Objective To explore the values of fetal fibronectin(fFN)and cervical length(CL)in the prediction of preterm birth in pregnant women with a history of spontaneous preterm birth.Methods The fFN in cervical secretions was detected by immunoassay and the CL was measured by abdominal ultrasonography in 50 singleton pregnant women with a history of spontaneous preterm birth.The incidence of spontaneous preterm birth was observed in pregnant women with fFN+(n=27),fFN-(n=23),CL≤25mm(n=29)or CL>25mm(n=21)before 35 weeks.Results Among the 50 pregnant women,spontaneous preterm birth occurred in 23(46.0%)before 35 weeks.The incidence of preterm birth in fFN+ pregnant women was significantly higher than that in fFN- pregnant women(P<0.001).The fFN+ was closely correlated with preterm birth(OR,19.05;95%CI,4.30-84.33,P<0.05).The incidence of preterm birth in pregnant women with CL≤25 mm is significantly higher than that in pregnant women with CL>25 mm(P<0.001).The risk for recurrence of preterm birth was high in pregnant women with CL≤25mm(OR,8.08;95%CI,2.13-30.58,P<0.05).Among pregnant women with CL≤25mm,the incidence of preterm birth in fFN+ women was significantly higher than that in fFN- women(P<0.001).Among pregnant women with CL>25mm,no significant difference in the incidence of preterm birth was found between fFN+ women and fFN- women(P>0.05).ConclusionThe combined fFN and CL detection has predictive value for preterm birth in pregnant women with a history of spontaneous preterm birth.更多还原
