【摘要】 目的探讨胰腺实性假乳头状瘤(solid-pseudopapil Iary tumor of pancreas,SPTP)的临床影像学特点,提高该病的术前诊断率。方法回顾性分析14例经手术及病理证实的胰腺实性假乳头状瘤的临床资料及其影像学特点。CT检查12例,MR检查2例。结果女性11例,男性3例,年龄22~62岁,平均年龄42岁。肿瘤位于胰头部2例,胰体部1例,胰尾部11例。肿瘤最大径为2.5~9.1cm,平均5.9cm,边界清楚。CT均表现为单发、外突性生长的囊实性肿块,其中2例以囊性成分为多,2例以实性成分为多,4例伴有钙化。CT增强扫描显示肿瘤实性部分呈渐进性强化,强化程度低于或接近正常胰腺实质,囊性成分各期均无强化。MR检查2例,1例在T1WI出现高信号,提示肿瘤内部有出血。结论胰腺实性假乳头状瘤的影像学表现具有一定的特征性,对术前制定手术方案,术后远期评估具有重要的临床价值。更多还原

【Abstract】 Objective To investigate the clinical imaging features of solid-pseudopapil Iary tumor of pancreas, and to improve the preoperative diagnosis rate of the disease. Methods Clinical data and features of imaging findings were retrospectively analyzed for 14 cases of solid-pseudopapillary tumor of pancreas, of which were confirmed by surgery and pathology. Of 14 patients 12 underwent CT and 2 did MR examination. Results Of 14 patients 11 were female and 3 were male, the age ranged from 22 to 62 years old with an average of 42 years. The lesions were respectively located in the pancreatic head in 2 cases, in the pancreatic body in 1 case and in the pancreatic tail in 11 cases. The maximal diameter of the tumors with clear boundary ranged from 2.5 cm to 9.1cm with a mean diameter 5.9 cm. The CT scan showed a single, round-like, cystic and solid mass in the pancreas. Of 14 cases 2 cases had more cystic elements, 2 did more solid elements, and 4 did calcification. After dynamic contrast-enhanced scanning, the solid parts of the lesion showed progressive enhancement and the cystic parts of the lesion did no enhancement. Of 2 cases underwent MR, 1 showed high signal intensity on T1-weighted imaging signified the bleeding in tumors. Conclusion Imaging findings of SPTP including CT and MR have certain characteristics. There have an important value for preoperative diagnosis, the formulate of operative plan and long-term postoperative assessment for the patients with SPTP.更多还原
