M2型丙酮酸激酶同工酶在人体早期非小细胞肺癌中的表达及临床意义

【摘要】 目的 研究M2型丙酮酸激酶同工酶(PKM2)在人体早期非小细胞肺癌(NSCLC)、癌旁正常肺组织及肺炎组织中的表达及临床意义。方法 收集外科手术切除并经病理确诊的非小细胞肺癌及癌旁正常组织标本以及纤维支气管镜肺活检病理检查确诊为肺炎的肺组织标本各99例,采用免疫组织化学法检测其PKM2的表达水平,并分析其表达与临床病理的关系。结果PKM2在NSCLC组织、癌旁正常肺组织和肺炎组织中的阳性率分别为69.70%、0%和9.09%。鳞状细胞癌明显高于腺癌(P<0.01)、腺鳞癌(P<0.05)和其他少见病理类型(P<0.01)。肿瘤直径>3 cm的PKM2表达率为76.92%,肿瘤直径≤3 cm的阳性表达率为42.86%,两组比较差异有统计学意义(P<0.01)。有淋巴结转移患者的PKM2表达率为87.18%,无淋巴结转移者为58.33%,两组比较差异有统计学意义(P<0.01)。Ⅰ期和Ⅱ期患者的PKM2阳性表达率分别为58.62%和85.36%,两组比较差异有统计学意义(P<0.01)。结论 PKM2在早期NSCLC中的表达率升高,对临床早期诊断NSCLC有一定的价值。

【Abstract】 Objective To explore the expression of M2 isoform of pyruvate kinase(PKM2) in early non-small cell lung cancer(NSCLC).normal lung tissue adjacent to carcinoma and pneumonia and related clinical significance.Methods Immunohistochemistry was used to detect the expression of PKM2 in NSCLC and normal lung tissue adjacent to carcinoma in 99 cases taken by surgical operation and pneumonia in 99 cases taken by fiberoptic bronchoscopy lung biopsy,and analyzed the relationship between PKM2 and the clinical characteristics.Results The positive rate of PKM2 expression was 69.70%,0%and 9.09%respectively in NSCLC,normal lung tissue adjacent to carcinoma and pneumonia.The positive rate of PKM2 was the biggest in squamous carcinoma group,adenosquamous carcinoma took the second place.The positive rate of PKM2 expression in patients with lymph node metastasis rate was 87.18%.significantly higher than that of patients without lymph node metastasis(58.33%),the difference was statistically significant(P < 0.01).The positive rate of PKM2 expression in patients with tumors greater than3 cm was 76.92%,significantly higher than that of patients with tumors smaller than or equal to 3cm(42.86%),the difference was statistically significant(P < 0.01).The positive rate of PKM2 was58.62%and 85.36%respectively in Ⅰ stage and Ⅱ stage patients,the difference was dramatically different(P<0.01).Conclusion The expression of PKM2 was significantly high in human early NSCLC,and it was valuable to early diagnosis of NSCLC.
