【摘要】 目的 检测原发性免疫性血小板减少症(ITP)患者血浆高迁移率族蛋白B1(HMGB1)、干扰素(IFN)-γ、白细胞介素(IL)-4和CD4+T细胞表面Toll样受体4(TLR4)表达的变化,研究其在ITP发病中的意义。方法 采用酶联免疫吸附试验法(ELISA)检测24例ITP患者和20例正常人血浆中HMGB1、IFN-γ、IL-4的水平,利用流式细胞仪分析外周血CD4+T细胞表面TLR4受体的表达率,分析HMGB1与TLR4及IFN-γ、IL-4之间的关系。结果 ITP患者血浆HMGB1水平明显高于正常对照组(P<0.05),IL-4水平明显低于正常对照组(P<0.05),IFN-γ水平与正常对照组比较差异无统计学意义(P>0.05);TLR4受体的表达明显高于正常对照组(P<0.05);HMGB1与IL-4、TLR4水平呈正相关(P<0.05),与IFN-γ水平呈负相关(P<0.05)。结论 ITP中存在血浆HMGB和CD4+T细胞表面TLR4受体的高表达,两者可能通过HMGB1/TLR4通路影响T淋巴细胞细胞因子的分泌。

【Abstract】 Objective To detect plasma levels of HMGB1,IFN-γ,IL-4 level and the expression of TLR4 on CD4+ T cells in patients with immune throbocytopenia(ITP) and then discussing their significance in ITP.Methods The concentrations of HMGB1,IFN-γ,IL-4 were detected by ELISA in 24 patients with ITP(ITP group) and 20 healthy people(control group).The expression of TLR4 were detected by flow cytometry as well.Then analyse the correlation between HMGB1 and IFN-γ,IL-4,the expression of TLR4.Results The HMGB1 level in ITP group were higher than those in control group(P <0.05),while the IL-4 level were lower in ITP group,and there was no difference of IFN-γ level between ITP and control groups(P > 0.05).The expression of TLR4 was higher in ITP group than in control group(P <0.05).There was a positive correlation between HMGB1 and IL-4,TLR4 expression(P < 0.05),and a negative correlation between HMGB1 and IFN-γ(P < 0.05).Conclusion There is a high expression level of plasma HMGB1 and TLR4 receptor in patients with ITP,and it is likely to affect T lymphocyte cytokine secretion through HMGB1/TLR4 pathways.
