载脂蛋白C3基因rs5128多态性与高甘油三酯血症关联性的荟萃分析

【摘要】 目的系统评价载脂蛋白C3(APOC3)基因rs5128多态性与高甘油三酯血症(HTG)的关联性。方法检索国内外相关数据库以获取所有公开发表的以探索APOC3基因rs5128多态性与HTG关联性的文献,截止时间2015年1月。根据纳入标准提取合格文献进行系统评价,采用显性、隐性、纯合子和等位基因遗传模型进行分析。根据异质性检验结果选择固定或随机效应模型进行数据合并,并采用Begg’s检验评价发表偏倚。结果共纳入18篇文献,包括病例1 462例,对照2 182例。在显性、隐性、纯合子和等位基因遗传模式下,APOC3基因rs5128多态性次要等位基因G与HTG的关联性有统计学意义(显性模型:OR=2.87,95%CI:1.99~4.14;隐性模型:OR=2.13,95%CI:1.43~3.19;纯合子模型:OR=2.98,95%CI:1.85~4.80;等位基因模型:OR=2.38,95%CI:1.77~3.20)。按人群来源进行亚组分析,在高加索和亚洲人群中G等位基因与HTG的关联性均有统计学意义,在高加索人群中的关联性更为密切。结论 APOC3基因rs5128多态性与HTG遗传易感性关联,需开展更多高质量、大样本的随机对照试验加以确定。

【Abstract】 Objective To investigate the association of apolipoprotein C3 gene( APOC3) rs5128 polymorphism with hypertriglyceridemia( HTG) by Meta-analysis. Methods The articles investigating the association between the APOC3 rs5128 polymorphism and HTG were retrieved from Chinese and international electronic databases. The published time was up to January 2015. The qualities of the included studies were evaluated and the data were extracted to analyze the association between APOC3 rs5128 polymorphism and HTG with dominant,recessive,homozygote and allele genetic models. The fix-effect or random-effect model was employed for data pooling according to the results of the heterogeneity test. Publication bias was evaluated with the linear regression of asymmetry Begg’ s test. Results Eighteen case-control studies containing 1 462 cases of HTG and 2 182 controls were included for the meta-analysis.The results showed that there were significant associations between the APOC3 rs5128 polymorphism and HTG with the dominant model( OR = 2. 87,95% CI: 1. 99-4. 14),recessive model( OR = 2. 13,95% CI: 1. 43-3. 19),homozygote model( OR = 2. 98,95% CI:1. 85-4. 80) and allelic model( OR = 2. 38,95% CI: 1. 77-3. 20). When stratified according to ethnicity,the significant association between the G allele of the APOC3 rs5128 polymorphism and HTG was present in both Caucasians and Asians,and the association was stronger in Caucasians than that in Asians. Conclusion The APOC3 rs5128 polymorphism may genetically associate with the risk of HTG. Further studies with high qualities and large samples should be needed to validate these findings.
