脐血血气分析结合胎儿血流动力学监测对围生期新生儿窒息的诊断意义

【摘要】 目的探究脐血血气分析结合胎儿血流动力学监测对围生儿窒息的诊断价值。方法随机选取2012年7月—2014年12月收治胎儿窘迫者100例(观察组)和健康剖宫产产妇100例(对照组),均行胎儿血流动力学监测和脐血血气分析,对比两组监测结果。结果观察组脐动脉S/D、RI、PI均高于对照组(P<0.05);观察组大脑中动脉各血流动力学指标均低于对照组(P<0.05)。观察组脐血p H、Pa O2均低于对照组,Pa CO2高于对照组(P<0.05)。结论胎儿血流动力学监测可预测胎儿宫内缺氧状况,脐血血气分析可判断新生儿的缺氧状况。

【Abstract】 Objective To explore the value of umbilical cord blood gas analysis combined with fetal hemodynamic monitoring in the diagnosis of perinatal neonatal asphyxia. Methods 100 puerperants with fetal distress and other 100 healthy puerperants underwent cesarean section from July 2012 to December 2014 were selected as the observation group and the control group, respectively. Fetal hemodynamic monitoring and umbilical cord blood gas analysis were performed in both groups. And the results of the two groups were compared. Results Compared with the control group, the observation group had higher values of umbilical artery S/D, RI, PI, P<0.05, lower values of middle cerebral artery hemodynamic parameters,P<0.05, lower values of p H and Pa OP but higher value of Pa CO2, P<0.05. Conclusion Fetal hemodynamic monitoring can predict intrauterine fetal hypoxia, umbilical cord blood gas analysis can determine the neonatal hypoxia.
