【摘要】 目的探析B超定位下舒芬太尼复合罗哌卡因应用于臂丛神经阻滞麻醉的效果。方法整群选取该院2012年1月—2015年3月收治的78例骨科手术患者,手术部位是单侧手部、腕部和前臂,将其随机分为A组、B组和C组,各为26例,A组麻醉给予罗哌卡因,B组麻醉给予罗哌卡因的同时复合舒芬太尼,C组麻醉给予罗哌卡因成功后,再静脉推注舒芬太尼,对比分析3组麻醉起效时间、维持时间及用药后各时间点的VAS评分。结果 3组在麻醉起效时间上差异无统计学意义(P>0.05);B组在麻醉维持时间上长于A组和C组(P<0.05);A组在用药后的6 h、12 h和24 h的VAS评分较B组和C组高(P<0.05);3组在注药后的0.5 h、1 h、3 h和48 h上的VAS评分差异无统计学意义(P>0.05)。结论 B超定位下舒芬太尼复合罗哌卡因应用于臂丛神经阻滞麻醉的效果显著,无不良反应,值得应用。更多还原

【Abstract】 Objective To investigate the effect of sulfentanyl and ropivacaine for brachial plexus block anesthesia with Bmode ultrasonography positioning. Methods 78 cases with orthopedic surgery admitted in our hospital from January 2012 to March 2015 were selected. The surgical sites of the patients were unilateral hand, wrist and forearm. The patients were randomly divided into group A, group B and group C with 26 cases in each. For anesthesia, patients in group A were given ropivacaine, those in group B were given ropivacaine and sulfentanyl, and those in group C were given intravenous injection of sulfentanyl after successfully giving ropivacaine. And the onset time of anesthesia, anesthesia duration, and the VAS scores at different time points after administration of the 3 groups were analyzed comparatively. Results The difference in the onset time of anesthesia between the three groups was not statistically significant, P>0.05. Group B had longer anesthesia duration than group A or group C, P<0.05. Group A had higher VAS scores than group B or group C 6h, 12 h, 24 h after administration, P<0.05. The differences in VAS scores half an hour, 1h, 3h, and 48 h after administration between the three groups were not statistically significant, P >0.05. Conclusion Ropivacaine combined with sulfentanyl with B-mode ultrasonography positioning for brachial plexus block anesthesia has significant effect without adverse reactions, which is worthy of application.更多还原
