【摘要】 目的探讨并分析陶瓷对陶瓷短柄全髋关节置换术在治疗中晚年股骨头坏死晚期患者的近期疗效。方法选取我院在2011-01~2013-12间收治的92例进行陶瓷对陶瓷短柄全髋关节置换术治疗中晚年股骨头坏死晚期的患者(共92髋),其中74例患者通过后外侧入路,另18例则通过前侧入路。在患者术后3个月、6个月、12个月以及之后每年进行一次随访,通过使用Harris髋关节评分和WOMAC骨关节炎指数对患者关节功能进行评估。结果 92例患者中共有90例患者进行了随访。最后一次随访中,患者拍摄的X光片显示假体位置和术后相比差异无统计学意义(P>0.05)。患者术后髋关节部位疼痛感明显减轻或消失,患者Harris髋关节评分和WOMAC骨关节炎指数较手术前有了明显改善,差异具有统计学意义(P<0.05)。随访期间患者均未发生感染、骨折或其他严重并发症、假体感染等。结论陶瓷对陶瓷短柄全髋关节置换术能够最大限度地保留患者骨量,且置换假体在初期稳定性教好,使用全陶瓷进行置换术能够降低患者磨损率,近期疗效较为理想,患者发生相关并发症的发生率较低。更多还原

【Abstract】 Objective Ceramic short shank ceramic total hip arthroplasty branch podium in the treatment of patients with advanced femoral head necrosis. Methods Selecting 92 patients with advanced femoral head necrosis in the hospital from January,2011 to December,2013 who were treated with ceramic short shank ceramic total hip arthroplasty branch podium,of which 74 cases were through a lateral approach,and the other 18 cases were through the anterior approach. After 3 months,6 months and 12 months,follow- up was done on the patients’ joint function by using the Harris hip score and WOMAC Osteoarthritis Index. Results Among 92 patients,90 patients were followed up. In the last follow- up,X- rays showed there was no significant difference of the prosthesis in patients with shooting position and before operation and postoperative( P > 0. 05). Postoperative pain in the hip joints significantly reduced or disappeared,the patient Harris hip score and WOMAC Osteoarthritis Index improved significantly compared with before surgery,and the difference was statistically significant( P < 0. 05). There was no infection,fracture,or other serious complications,prosthesis infection during follow- up patients. Conclusion Ceramic short shank ceramic total hip arthroplasty branch podium can protect bone mass maximumly,and the stability of prosthesis is relatively good. Ceramic replacement can reduce wear rate,the short- term effect is more ideal,and there is lower incidence of associated complications.更多还原
