肩袖撕裂磁共振成像与直接法磁共振关节造影的对比研究
Comparative study of magnetic resonance imaging and magnetic resonance arthrography in rotator cuff tears
【摘要】 目的探讨肩关节常规磁共振成像(magnetic resonance imaging,MRI)和直接法磁共振关节造影(magnetic resonance arthrography,MRA)对肩袖撕裂的诊断价值。方法分析36例临床诊断肩袖撕裂患者的常规MRI及MRA图像,以肩关节镜手术和开放手术结果为金标准,比较2种方法对肩袖撕裂的敏感度、特异度和准确度。结果关节镜手术和开放手术诊断肩袖全层撕裂15例,部分撕裂12例,无撕裂9例;MRI诊断肩袖全层撕裂11例,部分撕裂18例,无撕裂7例;MRA诊断全层撕裂16例,部分撕裂11例,无撕裂9例。MRI诊断肩袖撕裂的敏感度、特异度和准确度分别为93.1%、71.4%和88.9%;MRA诊断肩袖撕裂的敏感度、特异度和准确度均为100%结论 MRA是诊断肩袖撕裂特异度和准确度较高的方法。
【Abstract】 Objective To compare the diagnostic value of conventional magnetic resonance imaging(MRI)and magnetic resonance arthrography(MRA)in the detection of rotator cuff tears.Methods The conventional MRI and MRA shoulder examinations were performed in 36 patients with suspected rotator cuff tears,who subsequently underwent arthroscopy or open surgery.MRA or open surgery were used as the gold standard,and the diagnostic sensitivity,specificity and accuracy of two methods were compared.Results Based on the findings of arthroscopy or open surgery,15 cases were full-thickness tears,12 cases partial-thickness tears,and 9cases without tear.MRI diagnosis confirmed full-thickness rotator cuff tears in 11 cases,partial-thickness tears in 18 cases,and no tears in 7cases,MRA diagnosis proved full-thickness rotator cuff tears in 16 cases,partial-thickness tears in 11 cases,and no tears in 9cases.For the detection of the rotator cuff tears,the sensitivity,specificity,and accuracy of MRI were 93.1%,71.4% and 88.9%,respectively.For the detection of the rotator cuff tears,the sensitivity,specificity,and accuracy of MRA were 100%,100% and 100%,respectively.Conclusion Compared with MRI method,MRA has the good diagnostic specificity and accuracy for detection of rotator cuff tears.
