【摘要】 目的:比较磁共振成像(MRI)与关节镜对膝关节半月板损伤的诊断价值。方法:选择本院于2014年1-6月收治的78例临床诊断为膝关节半月板损伤患者作为研究对象,均行磁共振成像检查、关节镜检查,以关节镜检查结果为诊断金标准,统计MRI诊断半月板撕裂灵敏度、特异度等相关指标,评估其诊断价值。结果:43例患者为内侧半月板损伤,35例患者为外侧半月板损伤,MRI与膝关节镜检查无明显差异,P>0.05;MRI检查内侧半月板灵敏度为89.7%,特异度为85.7%,阳性预测值为92.9%,阴性预测值为80.0%,误诊率为14.3%,漏诊率为10.3%,诊断符合率为88.4%。MRI检测外侧半月板灵敏度为90.5%,特异度为92.9%,阳性预测值为95.0%,阴性预测值为86.7%,误诊率为7.1%,漏诊率为9.5%,诊断符合率为91.4%。结论:磁共振成像检查膝关节半月板损伤较为准确,具有重要诊断价值。更多还原

【Abstract】 Objective:To compare the diagnostic value of magnetic resonance imaging(MRI)and arthroscopy for meniscus injury in knee joint.Methods:A total of 78 patients with clinical diagnosis of meniscus injury in knee joint were selected,and all patients underwent MRI and arthroscopy.The results of arthroscopy were taken into consideration as the gold standard in the diagnosis for the statistic data of sensitivity,specificity and other related statistical indexes of MRI diagnosis of meniscal tear so as to evaluate its diagnostic value.Results:Of the 78 cases,43cases of patients were diagnosed as medial meniscus injury and 35 cases as lateral meniscus injury,there was no significant difference in MRI and arthroscopy for knee meniscus,P>0.05;By MRI examination,the sensitivity was 89.7%,specificity was 85.7%,the positive predictive value was 92.9%,the negative predictive value was 80%,the misdiagnosis rate was 14.3%,the rate of missed diagnosis is 10.3%,the diagnostic accordance rate was 88.4%.Sensitivity of MRI for detection of lateral meniscus was 90.5%,the specificity was 92.9%,the positive predictive value was 95%,the negative predictive value was 86.7%,the misdiagnosis rate was 7.1%,the rate of missed diagnosis was 9.5%,the diagnostic accordance rate was 91.4%.Conclusion:MRI examination for theknee meniscus injury is more correct and has more important value.更多还原
