增强型体外反搏联合高压氧治疗不稳定型心绞痛的临床疗效
Therapeutic effect of enhanced external counterpulsation combined with hyperbaric oxygen on unstable angina pectoris
【摘要】 目的观察增强型体外反搏(EECP)联合高压氧治疗不稳定型心绞痛(UAP)的临床疗效。方法接受EECP联合HBO治疗的UAP患者49例为联合治疗组,单纯接受EECP治疗的UAP患者49例为EECP组,标准内科治疗的UAP患者49例为对照组,比较3组治疗效果、心绞痛发作频率、持续时间及心肌缺血总负荷。结果联合治疗组总有效率为95.74%,EECP组为82.22%,对照组为61.7%,联合治疗组临床疗效优于对照组(P<0.05),联合治疗组与EECP组,EECP组与对照组疗效比较,差异无统计学意义(P>0.05)。治疗前,3组心绞痛发作频率、持续时间及心肌缺血总负荷比较差异无统计学意义(P>0.05)。治疗后,3组患者的心绞痛发作频率、持续时间及心电图心肌缺血总负荷均较治疗前下降(P<0.01),心绞痛发作频率联合治疗组<EECP组<对照组(P均<0.05);心绞痛持续时间3组间差异无统计学意义(P>0.05);联合治疗组及EECP组心肌缺血总负荷均低于对照组(P<0.05),联合治疗组与EECP组间差异无统计学意义(P>0.05)。结论与标准内科或单独EECP治疗方案相比,EECP联合HBO治疗UAP能显著减少心绞痛发作频率。
【Abstract】 Objective To observe the therapeutic effect of enhanced external counterpulsation( EECP) combined with hyperbaric oxygen( HBO) on unstable angina pectoris( UAP). Methods Forty-nine UAP patients in the combination therapy group received EECP combined with HBO,49 UAP patients in the EECP group were treated with EECP alone,and 49 UAP patients in the control group underwent standard medical therapy. A comparative analysis on therapeutic effects,frequency and duration of angina pectoris attacks,and overall myocardial ischemia loading was performed. Results The total effective rates of combination therapy group,EECP group and control group were 95. 74%,82. 22% and 61. 7%,respectively. The combination therapy group achieved better therapeutic effects compared with control group( P < 0. 05). There was no significant difference in the therapeutic effects among combination therapy group,EECP group and control group( P > 0. 05). The frequency of angina pectoris attacks,duration of angina pectoris attacks or overall myocardial ischemia loading showed no significant difference among three groups before treatment( P > 0. 05). The frequency and duration of angina pectoris attacks as well as the overall myocardial ischemia loading decreased among three groups after treatment compared with those before treatment( P < 0. 01).The combination group gained the lowest frequency of angina pectoris attacks while control group obtained the highest one among three groups( all P < 0. 05). The duration of angina pectoris attacks showed no significant difference among three groups( P > 0. 05). The overall myocardial ischemia loading was lower in the combination therapy group and EECP group compared with that in the control group( P < 0. 05),which showed no significant difference between combination therapy group and EECP group( P > 0. 05). Conclusion Compared with EECP therapy or standard medical therapy,EECP combined with HBO for the treatment of UAP might significantly decrease the frequency of angina pectoris attacks.
