不同缺血预调适方案对大鼠肢体骨骼肌缺血再灌注损伤的影响

【摘要】 目的:探讨不同缺血预调适(IP)方案对大鼠肢体骨骼肌缺血再灌注(IR)损伤的影响。方法:将48只健康成年雄性SD大鼠随机分为IR组、IP A组、IP B组、IP C组,每组12只,各组均予股动脉夹闭4h,再复灌最长24h,IP 3组于4h缺血前施以3个不同方案的预处理:IP A组:6个循环2min缺血/2min灌注;IP B组:3个循环5min/缺血5min灌注;IP C组:10个循环1min缺血/1min灌注。各组骨骼肌IR后2h、12h、24h检测血清肌酸激酶(CK)、乳酸脱氢酶(LDH)含量、骨骼肌湿干比(W/D)值、骨骼肌肌肉坏死面积以及骨骼肌组织形态、超微结构变化。结果:IP A组、IP B组、IP C组骨骼肌IR 2h、12h、24h的血清CK及LDH含量上升、骨骼肌W/D值增加、骨骼肌肌肉坏死面积、骨骼肌组织学改变及超微结构变化,均比IR组有明显好转,差异均有统计学意义(P<0.05)。结论:IP方案能减轻大鼠肢体IR损伤,以3个循环5min缺血/5min灌注的IP方案保护作用最佳。

【Abstract】 Objective:To investigate effect of different protocols of ischemic preconditioning on skeletal muscle ischemia reperfusion injury in a rat model.Methods:Forty-eight healthy adult male SD rats were randomly divided into ischemia reperfusion group(IR),ischemic preconditioning(IP)of ischemia groups(Group A,B and C).Each group and subgroups had 12 rats.All rats were subjected to 4-hour ischemia via occlusion of the femoral artery,followed by 24 hours of reperfusion.Additional ischemic preconditioning interventions were applied in IP Group A,B and C,which before the 4-hour ischemia with three different protocols respectively:6cycles of 2min ischemia/2min reperfusion(IP Group A);3cycles of 5min ischemia/5min reperfusion(IP Group B);and 10 cycles of 1min ischemia/1min reperfusion(IP Group C).At2-,12-and 24-hour reperfusion,blood samples were taken for detecting serum CK、LDH,and muscle tissue samples were taken for measuring wet/dry ratio,necrosis area,histology and ultrastucture changes.Results:CK,LDH,W/D ration,necrosis area,histology and ultrastructure changes in IP group A,B and C were significantly better than those in IR group(P <0.05).Conclusion:Protocol of IP can confer protection from skeletal muscle ischemia-reperfusion injury in the rat model,and 3cycles of 5min ischemia/5min reperfusion protocol is optimal.
