影响献血者ALT初筛结果的原因分析
Analysis of factors affecting results of ALT preliminary screening in blood donors
【摘要】 目的探讨采用干化学法初筛献血者丙氨酸氨基转移酶(ALT)的影响因素,以有效建立初筛ALT标准,减少血液浪费。方法收集2013年1~6月茂名市中心血站无偿献血者血样标本21 065份,对比测试标本溶血、脂血、加样量、加样后等待时间等因素对干化学法检测ALT的影响,并根据结果规范操作,设定适宜的初筛限值标准。结果标本溶血对ALT检测结果产生影响,溶血率小于或等于0.5%对ALT检测无明显影响;脂血对检测结果无明显影响;加样量为(32±3)μL、加样后等待时间为10~40s,ALT检测结果可接受。设定初筛限值标准为ALT≤40U/L(女),ALT≤45U/L(男)。结论标本溶血、加样量、加样后等待时间对干化学法检测ALT产生影响,而脂血对检测结果无明显影响,规范操作、合理设置正常限值可保证检测的准确性和稳定性,节约血液资源,进一步保证血液安全,值得广泛应用和推广。
【Abstract】 Objective To explore the factors affecting results of alanine aminotransferase(ALT)preliminary screening by using dry chemical method in blood donors,so as to establish standards of ALT preliminary screening and reduce waste of blood.Methods A total of 21 065 blood specimen of blood donors in Blood Center of Maoming City were collected from January to June2013.The influences of specimen hemolysis,lipidemia,different sample amount and waiting time after adding sample on the detection results of ALT by using dry chemical method were comparatively analysed,and standardized procedures and established appropriate limits of ALT preliminary screening according to the results.Results The specimen hemolysis affected test results of ALT,which was not obvious when the hemolysis rate was less than or equal to 0.5%.There was no obvious interference of lipidemia on ALT test.When the sample amount was(32±3)μL or the waiting time after adding sample was 10 to 40seconds,the detection results of ALT were acceptable.The standard limit of ALT was less than or equal to 40U/L of female and less than or equal to 45U/L of male.Conclusion Hemolysis,sample amount and waiting time after adding sample can influence the results of ALT detected by using dry chemical method,while lipidemia had no interference on ALT test.Standardizing operation and reasonably setting normal limits standard may ensure the accuracy and stability of determination,save blood resources,further ensure blood safety,which should be worthy of wide application.
