早期2型糖尿病人群身体成份对骨密度的影响

【摘要】 目的探讨身体成份对中老年2型糖尿病患者骨密度(bone mineral density,BMD)的影响。方法选取为2012年内分泌科住院2型糖尿病患者224例(男性115例、女性109例),年龄40~75岁、病程不超过5年、未使用胰岛素治疗且无明显糖尿病并发症。研究对象均进行体检,检测糖化血红蛋白(glycosylated hemoglobin,Hb A1c,Simens,DCA Wantege)、BMD及身体成份(美国Hologic公司,ASY-00409双能X线骨密度仪)。结果瘦组织与股骨颈、髋部、脊柱BMD呈正相关(相关系数r分别为0.61、0.64、0.60,P值均小于0.05);校正年龄、Hb A1c、体质量指数(body mass index,BMI)后,体脂含量与女性脊柱(L1-4)BMD呈正相关(相关系数r=0.24,P<0.05),与男性BMD无明显相关性;腹型肥胖(Android)与股骨颈、髋部、脊柱(L1-4)BMD呈正相关(r分别为0.27、0.32、0.26,P均<0.05)。结论瘦组织对BMD产生有利影响,体脂含量对绝经女性产生有利影响,而对男性BMD则无保护作用。

【Abstract】 Objective To explore the influence of body composition on bone mineral density( BMD) among middle-aged and elderly diabetics. Methods Subjects for this cross-sectional analysis were men( n = 115) and women( n = 109) aged 40- 75 years( 59 ± 9) with uncomplicated,course of disease less than 5 years,noninsulin-required type 2 diabetes. Blood pressure,height,weight,waist circumference,body mass index( BMI) was measured. Glycosylated hemoglobin( Hb A1c) were tested( Simens,DCA Wantege). BMD and body composition were measured( dual energy X-ray absorptiometry Hologic, ASY-00409). Results Lean mass positively correlated with BMD at femoral neck,hip and lumbar spine L1-4;( r = 0. 61,0. 64,0. 60,P < 0. 05). After adjusted with BMI,Hb A1 c and age,fat mass positively correlated with BMD at lumbar spine L1-4in women( r = 0. 24,P < 0. 05),however,no obvious correlation with BMD in men. Android positively correlated with BMD at femoral neck,hip and lumbar spine( r = 0. 27,0. 32,0. 26,P < 0. 05). Conclusion Lean mass has positive effect on BMD. Fat mass has positive effect on BMD in women,but has no effect on BMD in men.
