【摘要】 目的通过测量唇腭裂患者手术辅助快速上颌扩弓前后头颅侧位片及模型的相关数据,观察扩弓治疗对患者牙颌系统的影响。方法选择11例处于生长发育期的单侧完全性唇腭裂患者,进行手术辅助快速上颌扩弓,扩弓前后拍摄锥形束计算机断层扫描,由软件生成侧位片后进行头影测量分析,制取石膏模型并测量其变化。采用配对t检验进行统计分析。结果头影测量及模型分析显示扩弓后上切牙轴倾度减小,上下第一磨牙垂直向压低,牙弓宽度和牙槽骨宽度增加,与扩弓前比较,差别有统计学意义(P<0.05);下颌平面角及腭平面角扩弓前后比较,差别无统计学意义(P>0.05)。结论扩弓前后牙颌系统改变主要在横向上,矢状向和垂直向上改变较小。生长发育期的唇腭裂患者进行手术辅助扩弓可以达到较为满意的临床效果。更多还原

【Abstract】 Objective The study was to examine the skeletal and dental changes after surgically assisted rapid palatal expansion in individuals with unilateral cleft lip and palate by cephalometric and model analysis. Methods The sample consisted of 11 adolescents with unilateral complete cleft lip and palate. They all underwent surgically assisted rapid palatal expansion. Expansion results were evaluated by cephalograms derived from CBCT images and models taken before and after expansion. Paired t-test was performed for statistical analysis. Results The results showed that from pretreatment to completion of expansion:inclination of upper incisor had significantly decreased;the upper first molar and lower first molar were significantly intruded;both the width of dental arch and alveolar significantly increased(P<0.05);the maxillary plane and mandibular plane had no significant rotation(P>0.05). Conclusion There were statistically significant differences in transverse dimension,not in the sagittal and vertical.Surgically assisted rapid palatal expansion in adolescents with cleft lip and palate can produce satisfactory clinical effect.更多还原
