顺式阿曲库铵对丙泊酚浅麻醉下听觉诱发电位指数的影响
The Effect of Cisatracurium on Auditory Evoked Potential Index Undergoing Light Anesthesia with Propofol
【摘要】 目的观察不同剂量顺式阿曲库铵对丙泊酚浅麻醉状态下听觉诱发电位指数(AAI)的影响。方法将择期手术全麻患者80例随机分为对照组(C组)、2ED95(E2组)、4ED95(E4组)及8ED95(E8组)顺式阿曲库铵组4组,分别于诱导后将丙泊酚效应室靶浓度设为2.5μg/mL,稳定后C组静脉注射生理盐水5mL;E2、E4、E8组分别静脉注射2ED95(0.1mg/kg),4ED95(0.2mg/kg),8ED95(0.4mg/kg)顺式阿曲库铵。注药后观察15min停止试验,记录注药前5min(T0)、注药后第1个5min(T1)、第2个5min(T2)及第3个5min(T3)内各实测听觉诱发电位指数(AAI)、肌电图(EMG)平均值。结果与T0相比,E2、E4、E8各组AAI值在T1、T2、T3时段内均降低,差别有统计学意义(P<0.05);与C组比较,E2、E4、E8组注药后各时段AAI值均显著降低(P<0.05)。EMG在E2、E4、E8组T1、T2、T3时段内与T0相比均显著降低(P<0.001)。结论浅麻醉状态下,静脉注射顺式阿曲库铵可使AAI值显著下降,下降的时段与肌松药的药代动力学相关;不同剂量间下降幅度差别无统计学意义。
【Abstract】 Objective To observe the effect of different doses of cisatracurium on auditory evoked potential index undergoing light anesthesia. Methods 80 patients undergoing elective operation with general anesthesia were randomly assigned to control group(group C),2ED95(group E2),4ED95(group E4),8ED95(group E8),20 cases each group. The cisatracurium concentrations for group E2,group E4,and group E8 were 0.1mg/kg,0.2mg/kg,and 0.4mg/kg respectively. 5mL of saline or cisatracurium injection was administered while the effect target concentration of propofol was set at 2.5μg/mL. The patients were observed for 15 min before the test ended. Average value of AAI and EMG were recorded at time of before injection(T0),and first 5min(T1),second 5min(T2),third 5min(T3)after injection. Results Compared with baseline(T0),values of AAI in T1-3period were decreased in cisatracurium-treated groups(P<0.05). Compared with control group,value of AAI in cisatracurium-treated groups in T1-3period was decreased(P<0.05). Compared with T0,values of EMG in T1-3period were decreased in cisatracurium-treated groups(P<0.001). Conclusion Value of AAI can be decreased,with a statistical significance,after cisatracurium injection undergoing light general anesthesia. The difference in AAI decrease among different doses of cisatracurium was not statistically significant though.
