【摘要】 目的探讨发育性髋关节脱位(DDH)筛查的临床应用价值。方法选择温岭市妇幼保健院2012年1月至2014年12月经儿保门诊初步筛查的4905例1周岁以内临床可疑婴儿,用髋关节数字化X线摄影(DR)检查进行DDH筛查,参照金汉珍、潘恩源和徐赛英等诊断标准进行影像学评估,对阳性患儿进行临床随访,结合临床随访结果对其DR表现特征进行回顾性分析。结果 4905例DDH筛查临床可疑婴儿中诊断为髋关节脱位22例,髋关节半脱位59例,髋关节发育不良69例,疑似髋关节半脱位46例,疑似髋关节发育不良179例。临床随访和复查361例DDH筛查婴儿,支架固定整复等治疗122例,蛙式抱养198例,未经支架固定等方法治疗或未按医嘱佩戴支架治疗而致病情恶化18例。结论 DDH患儿通过早期检查和早期诊断可以进行早期治疗,能降低其晚期发病率和致畸率,DDH筛查具有重要的临床应用价值。更多还原

【Abstract】 Objective To investigate the clinical application value of developmental dislocation of the Hip( DDH).Methods From January 2012 to December 2014,fourty nine hundrad and five cases clinical suspected babies by preliminary screening in wenling health hospital for women and children were selected. DDH was screened by the hip joint digital radiography( DR),and imaging assessment was based on the diagnostic criteria of Jin HZ,Pan EY and Xu SY. The positive children were followed-up,combined with clinical follow-up results,the DR features were retrospectively analyzed. Results Among the 4905 clinical suspected cases,22 cases had hip dislocation,59 cases had hip subluxation,69 cases had hip dysplasia,46 cases had suspected hip subluxation,and 179 cases had suspected hip dysplasia. There were clinical follow-up and review in 361 cases.There were fixation treatment for Von-Rosen bracket in 122 cases,Von-Rosen adoption in 198 cases. The disease was deteriorated in 18 patients caused by without treatment or not according to the prescribed treatment. Conclusion By the early examination and early diagnosis,early treatment can be carried out. It can reduce the late incidence and rate of abnormality of DDH in children,so it has important clinical application value in screening of DDH.更多还原
