【摘要】 目的:研究直接和延迟介入治疗对急性前壁心肌梗死患者左心室结构和功能的影响。方法:选取2012年1月~2013年1月我院收治的急性前壁心肌梗死患者70例,随机分为A组和B组,每组35例,A组实施直接行介入治疗,B组先进行溶栓治疗,然后后行介入治疗,随访患者1年,比较两组治疗前后患者的左心射血分数、室壁运动积分指数以及容量、再更死率。结果:A组总有效率和B组总有效率比较差异无统计学意义(P>0.05);治疗后A组射血分数、收缩末容量指数、舒张末容量指数以及室壁运动指数分别为(55.2±1.8)%、(27.1±0.5)ml/m2、(57.1±0.1)ml/m2和(1.1±0.1),B组射血分数、收缩末容量指数、舒张末容量指数以及室壁运动指数分别为(54.5±0.2)%、(28.3±1.1)ml/m2、(58.2±0.5)ml/m2和(1.2±0.3),两组治疗后射血分数、收缩末容量指数、舒张末容量指数以及室壁运动指数均优于治疗前,与治疗前比较差异具有统计学意义(P<0.05);两组间比较差异均无统计学意义(P>0.05),A组和B组半年内再梗死率比较无统计学意义(P>0.05)。结论:直接介入治疗和间接介入治疗急性前壁心肌梗死均具有一定的疗效,在不具备介入治疗技术的医院可以先行静脉溶栓治疗,后再选择有条件的医院进行介入治疗。更多还原

【Abstract】 Objective: To study the impact of immediate and delayed intervention in treatment of left ventricular structure and function of patients with acute anterior myocardial infarction.Methods:70 cases with acute anterior myocardial infarction were selected from our hospital during the period from January 2012 to January 2013,and they were randomly divided into A group and B group.There are 35 cases in each group.The patients of the A group were given immediate intervention,the patients of the B group were given thrombolytic therapy first,and then given intervention,patients were followed up for 1 year,the left ventricular ejection fraction before and after treatment,wall motion score index and capacity,death rates even more of two groups were compared.Results:The clinical effective rate of the A group and B group the difference between the two groups had no statistically significant(P>0.05),the ejection fraction,Endsystolic volume index,End-diastolic volume index and Wall motion index of A group after treatment were(55.2±1.8)%,(27.1±0.5)ml/m2,(57.1±0.1)ml/m2 and(1.1±0.1),the ejection fraction,End-systolic volume index,End-diastolic volume index and Wall motion index of B group were(54.5±0.2)%,(28.3±1.1)ml/m2,(58.2±0.5)ml/m2 and(1.2±0.3),the ejection fraction,End-systolic volume index,End-diastolic volume index and Wall motion index of two groups after treatment Are better than before treatment,the difference was statistically significant(P<0.05),The difference between the two groups had statistically significant(P<0.05);The death rates even more of the A group and B group had no statistically significant(P>0.05).Conclusion:Immediate and delayed intervention in treatment of acute anterior myocardial infarction had a certain effection,hospital without interventional therapy can Intravenous thrombolytic therapy first,then given intervention in Conditional Hospital.更多还原
