调补肺肾三法对慢性阻塞性肺疾病稳定期模型大鼠肺胶原和蛋白酶调节作用的R值综合评价

【摘要】 目的采用R值综合评价法多指标综合评价调补肺肾三法(补肺健脾、补肺益肾、益气滋肾)对慢性阻塞性肺疾病(COPD)稳定期大鼠肺胶原和蛋白酶的调节作用。方法采用香烟熏吸联合细菌感染法制备COPD稳定期大鼠模型,于实验第9周起分别给予补肺健脾方、补肺益肾方、益气滋肾方及氨茶碱灌胃治疗至第20周,于实验第20、32周检测大鼠肺胶原和蛋白酶的9个指标,包括Ⅰ型胶原、Ⅲ型胶原、Ⅳ型胶原、基质金属蛋白酶-2(MMP-2)积分光密度(IOD)、基质金属蛋白酶-9(MMP-9)IOD、金属蛋白酶组织抑制剂-1(TIMP-1)IOD,MMP-2 mRNA、MMP-9 mRNA、TIMP-1 mRNA。建立R值综合评价法,并以此对肺胶原和蛋白酶的调节作用进行综合评价。结果第20、32周及综合20、32周,调补肺肾三法和氨茶碱对肺胶原和蛋白酶综合9指标的纠正强度显著(P<0.01)。第20、32周,综合纠正强度依次为补肺健脾、补肺益肾、益气滋肾、氨茶碱,且第32周补肺健脾、补肺益肾的纠正强度较氨茶碱显著,补肺益肾的纠正强度较本组第20周显著(P<0.05)。综合20、32周,综合纠正强度依次为补肺健脾、补肺益肾、益气滋肾、氨茶碱,且补肺健脾和补肺益肾纠正强度较氨茶碱显著(P<0.05或P<0.05)。结论调补肺肾三法对COPD稳定期大鼠肺胶原和蛋白酶的综合调节作用显著,且具有较好的远后效应,以补肺健脾、补肺益肾效果更佳。

【Abstract】 Objective To evaluate the regulatory effect of three methods for lung-kidney regulating and supplementing( lung-supplementing spleen-strengthening method,lung-supplementing kidney-benefiting method and qi-benefiting kidney-nourishing method) on lung collagen and protease in stable chronic obstructive pulmonary disease( COPD) model rats. Methods The stable COPD rat models were established by smoke inhalation and bacterial infection. The rats were gavaged with lung-supplementing spleen-strengthening formula,lung-supplementing kidneybenefiting formula,qi-benefiting kidney-nourishing formula and aminophylline respectively from the 9th week to the 20th week of experiment. Nine indicators related to lung collagen and proteases including collagen type Ⅰ,collagen type Ⅲ,collagen type Ⅳ,matrix metalloproteinase-2( MMP-2) integrated optical density( IOD),MMP-9 IOD,tissue inhibitor of metalloproteinase-1( TIMP-1) IOD,MMP-2 mRNA,MMP-9 mRNA and TIMP-1 mRNA were detected in the 20th week and 32nd week. The regulatory effect on lung collagen and proteases was evaluated with R-value comprehensive evaluation. Results The nine indicators related to lung collagen and proteases were significantly rectified by three methods for lung-kidney regulating and supplementing and aminophylline in the 20th week,32nd week and both the 20th and 32nd week( P < 0. 01). In the 20th week and 32nd week,the comprehensive rectifying intensity from high to low was lung-supplementing spleen-strengthening,lung-supplementing kidney-benefiting method,qi-benefiting kidney-nourishing method and aminophylline,especially lung-supplementing spleen-strengthening method and lung-supplementing kidney-benefiting method as compared with aminophylline in the 32nd week( P < 0. 05) and lung-supplementing kidney-benefiting method in the 32nd week as compared with in the 20th week( P < 0. 05). Considering both the 20th and 32nd week,the comprehensive rectifying intensity from high to low was lungsupplementing spleen-strengthening method, lung-supplementing kidney-benefiting method, qi-benefiting kidneynourishing method and aminophylline, especially lung-supplementing spleen-strengthening method and lungsupplementing kidney-benefiting method as compared with aminophylline( P < 0. 05). Conclusion Three methods for lung-kidney regulating and supplementing can obviously regulate the lung collagen and protease in COPD rats with a better long-term effect,especially lung-supplementing spleen-strengthening method and lung-supplementing kidneybenefiting method.
