【摘要】 目的探讨缺血性中风急性期证候与大动脉病变(LVD)、小动脉病变(SVD)的相关性。方法将207例缺血性中风急性期患者分为LVD组87例、SVD组120例,比较两组患者的中医证候、病史、并发症、血常规、颅内外血管超声学检查、美国国立卫生研究院卒中量表(NIHSS)、改良Rankin量表(MRS)及日常生活活动能力量表(ADL)评分,并采用logistic回归对相关因素进一步分析。结果两组患者痰证单项条目评分、阴虚阳亢证发生率、房颤、冠心病、心脏病史、心律失常并发症、淋巴细胞百分比(LYM%)、高密度脂蛋白(HDL)、低密度脂蛋白(LDL)、TCD血管高流速、NIHSS、MRS量表评分差异有统计学意义(P<0.05),并采用logistic回归对上述因素进一步分析得出,冠心病史、LDL、TCD血管高流速、MRS量表评分与LVD相关(P<0.05),LDL与SVD相关(P<0.05)。结论缺血性中风急性期大小动脉病变表现出不同的证候特征。更多还原

【Abstract】 Objective To study the correlation of patterns,indicators,large vessel disease( LVD) and small vessel disease( SVD) in acute ischemic stroke. Methods Totally 207 cases of acute ischemic stroke were divided into the LVD group and SVD group,with 87 and 120 in each. The patterns,medical history,complications,routine blood test,ultrasound examination of intracranial vascular and scores of National Institutes of Health Stroke Scale( NIHSS),modified Rankin Scale( MRS) and activities of daily living( ADL) scale were compared in both groups. The related factors were analyzed with logistic regression. Results There were significant differences between groups in the score of phlegm pattern,incidence of yin deficiency yang hyperactivity pattern,atrial fibrillation,coronary heart disease( CHD),history of heart disease,arrhythmia,lymphocyte ratio( LYM%),high-density lipoprotein cholesterol( HDL-C),low-density lipoprotein cholesterol( LDL-C),high Transcranial Doppler( TCD) blood flow velocity,NIHSS score and MRS score( P < 0. 05). CHD history,LDL,high TCD blood flow velocity and MRS score were related to LVD( P < 0. 05). LDL was related to SVD( P < 0. 05).
