SPECT/CT心肌灌注显像图像配准情况及其对图像质量的影响

【摘要】 目的探讨SPECT/CT心肌灌注显像CT与SPECT图像的配准及配准不良对CT衰减校正(CTAC)图像质量的影响。方法分析110例99Tcm-甲氧基异丁基异腈SPECT/CT心肌灌注显像受检者资料,利用仪器自带的图像融合软件观察CT和SPECT图像的配准情况,并对出现配准不良的受检者图像行重新配准。利用靶心图获得左心室各壁的放射性计数百分比,比较不同方向配准不良与重新配准后左心室各壁放射性计数的差异和图像差异。结果有58例(58/110,52.73%)共出现78次图像配准不良,以x轴方向向右偏移和z轴方向向下偏移最常见。对配准不良的图像重新配准后,前壁、心尖部和侧壁的放射性计数百分比明显增加(t=7.931、2.385、8.320,P均<0.05),其中x轴方向的配准不良对侧壁的影响最为显著,z轴方向的配准不良对前壁的影响最为显著。图像重新配准后可明显改善左心室前壁、心尖部和侧壁的放射性分布。结论图像配准不良以x轴方向向右偏移和z轴方向向下偏移最为常见,对前壁、心尖部和侧壁的影响最为显著;对CT和SPECT图像行重新配准后可明显改善左心室各壁的图像质量。

【Abstract】 Objective To explore the effect of registration and misregistration of CT and SPECT images on image quality of CT attenuation correction(CTAC) in SPECT/CT myocardial perfusion imaging.Methods Totally 110 patients performed rest 99Tcm-methoxy isobutyl isouitril(99Tcm-MIBI) myocardial perfusion SPECT and underwent CTAC on a SPECT/CT system.The circumstances of image misregistration between SPECT and CT were observed by using the image function software,and reregistrationed of CT and SPECT images for the misregistration data were analyzed.Then counts percentage of radioactivity of the left ventricle using myocardial Bull’s-eye were measured.The different directions misregistration differences and image differences reregistrationed of the left ventricular walls radioactive counts were compared.Results All of 58 patients(58/110,52.73%) showed 78 misregistration,and it was frequently occurred in x-axes that motion to right and in z-axes that motion to down.The counts percentage of radioactivity of anterior,apex and lateral wall was significant increased when the misregistration images were reregistrationed(t=7.931,2.385,8.320,all P<0.05),the misregistration in x-axes was most impact on the lateral wall and z-axes was most impact on the anterior wall.The image quality could be obviously improved after reregistrationed expressly in anterior,apex and lateral wall.Conclusion The image misregistration is frequently occurred in x-axes that motion to right and in z-axes that motion to down,which is mostly impact on the anterior,apex and lateral wall.The image quality could be obviously improved after reregistrationed.
