精神分裂症患者血清同型半胱氨酸与注意及执行功能的相关性
Correlation of attention and executive functioning to serum homocysteine level in patients with schizophrenia
【摘要】 目的:探讨血清同型半胱氨酸(Homocysteine,Hcy)与精神分裂症注意及执行功能的相关性。方法:选取门诊及住院精神分裂症患者118人,在该院康复科社会志愿者中选取健康者72例为正常对照组。测定空腹静脉血清同型半胱氨酸(>15μmol/L为高Hcy)、叶酸、维生素B12含量;采用连续操作测验(CPT)评定持续注意能力,威斯康星卡片分类测验(WCST)评定执行功能。结果:患者组血清Hcy值高于正常对照组[(21.8±7.7)μmol/L vs.(15.4±5.2)μmol/L,P<0.05],男性患者血清Hcy值高于女性患者[(27.9±4.4)μmol/L vs.(16.5±3.5)μmol/L,P<0.05]。患者组中高血清Hcy者共61人(51.69%),男性多于女性(61.6%vs.40.4%,P<0.05)。患者组的CPT和WCST测验得分均低于正常对照组,高Hcy患者的CPT和WCST测验得分均低于正常Hcy患者(均P<0.05)。患者组的血清Hcy值与CPT正确反应数及WCST正确测验数、分类完成数得分均呈负相关(r=-0.83、-0.78、-0.74,均P<0.01),而与CPT干扰项错误数及WCST持续错误数、随机错误数得分呈正相关(r=0.65、0.84、0.78,均P<0.01)。结论:本研究提示,精神分裂症患者的血清Hcy值异常升高与其注意及执行功能密切相关。
【Abstract】 Objective: To explore the relationship between serum homocysteine level and attention and executive dysfunction in patients with schizophrenia. Methods: The serum homocysteine,folate and vitamin B12 level of fasting venous blood were tested( serum homocysteine values > 15μmol / L were identified as high serum homocysteine) in patients with schizophrenia( n = 118) and controls( n = 72). Attention function and executive function in all participants were evaluated with the Continuous Performance Test( CPT) and Wisconsin Card Sorting Test( WCST). Results: The serum homocysteine levels were higher in the patients than that in the controls [( 21. 8 ±7. 7) μmol / L vs.( 15. 4 ± 5. 2) μmol / L,P < 0. 05],and higher in males than females [( 27. 9 ± 4. 4) μmol / L vs.( 16. 5 ± 3. 5) μmol / L,P < 0. 05]. The rate of high homocysteine in male schizophrenia( 61. 6%) was higher than female( 40. 4%)( P < 0. 05). There were significantly different between patients and healthy people in the scores of CPT and WCST( P < 0. 05), and also significantly different between patients with high serum homocysteine and normal serum homocysteine level( P < 0. 05). The serum homocysteine levels were negatively correlated with the correct responses scores of CPT,the correct test number and classification completion of WCST( r =- 0. 83,-0. 78,- 0. 74,Ps < 0. 01),and positively correlated with distractor errors of CPT,the persistent errors and random errors of WCST( r = 0. 65,0. 84,0. 78,Ps < 0. 01). Conclusion: The abnormally higher serum homocysteine level may be closely correlated with the attention and executive function in patients with schizophrenia.
