【摘要】 目的提高医疗服务质量,优化调剂服务,保证用药安全。方法针对门诊调剂中遇到的退药问题进行统计、分析,并提出合理性解决方案。结果医师处方行文不规范是造成退药的主要原因,占退药总数的61.68%,经过干预后,退药处方占全年总处方的比率明显下降,由5.27‰下降至1.72‰。结论临床医师及药师应加强药学、医学知识的培训和学习,减少门诊退药,提高门诊用药安全性。更多还原

【Abstract】 Objective To improve medical service quality,optimize swap service and to ensure the safety use of drugs.Methods To statistics and analysis the drug withdrawal problem and propose reasonable solutions.Results Physician’s error was the main reason and its rate was 61.68%.By taking interventions,the rate of the drug withdrawal in total annual prescription was reduced from 5.27‰ to 1.72‰.Conclusion Clinicians and pharmacists should strengthen the learning of pharmaceutical and medical knowledge to enhance outpatient medication safety.更多还原
