慢性乙型肝炎患者外周血程序性死亡分子-1的表达特点
Expression characteristics of programmed cell death-1 in chronic hepatitis B patients
【摘要】 目的分析慢性乙型肝炎患者(CHB)CD4+T细胞中程序性死亡受体-1(programmed death-1 PD-1)和配体(Programmed cell death ligand,PD-L1/B7-H1)的表达水平与HBV DNA和HBeAg水平的关系。方法运用流式细胞术检测54例慢性乙肝患者和27例健康对照组外周血中CD4+T细胞上的PD-1/PD-L1的表达频率。结果 54例CHB患者的PD-1与PD-L1的表达水平分别为0.96%±0.29%和0.49%±0.18%,显著高于健康对照组(t分别为7.89和6.57,P<0.01),HBeAg阳性组患者外周血T细胞中PD-1的表达频率显著高于阴性组(1.04%±0.29%vs0.88%±0.25%,t=2.85,P<0.01)。HBeAg阳性组患者PD-L1百分率显著高于阴性组(0.51%±0.12%vs 0.43%±0.12%,t=2.26,P<0.05)。病毒载量与PD-1和PD-L1的表达水平呈显著正相关(r分别为0.393和0.271,P<0.01),与ALT、TBIL、PT无相关性。结论慢性乙型肝炎患者淋巴细胞上B7-HI和PD-1的表达水平与患者疾病状态密切相关,可能是慢性乙型肝炎患者治疗的潜在靶点。
【Abstract】 Objective To analyze the relationship between the expression levels of PD- 1( programmed death receptor- 1) and its ligand PD- L1( programmed cell death ligand PD- L1 /B7- H1) in CD4+T cells and HBeAg and HBV DNA in chronic hepatitis B patients( CHB). Methods The expression levels of PD- 1 and PD- L1 on CD4+T cells of 54 CHB patients and27 healthy controls were measured by flow cytometry. Results The levels of PD- 1 and PD- L1 in 54 CHB patients were0. 96% ± 0. 29% and 0. 49% ± 0. 18%,significantly higher than those in healthy controls( t = 7. 89 and 6. 57,respectively,all P < 0. 01). PD- 1 expression frequency on T cells of peripheral blood of HBeAg positive patients was 1. 04 % ± 0. 29%,and that of HBeAg negative patients was 0. 88 % ± 0. 25%,the difference had statistical significance( t = 2. 85,P < 0. 01). PD-L1 level in HBeAg positive group was higher than that in HBeAg negative group( 0. 51% ± 0. 12% vs 0. 43% ± 0. 12%,t =2. 26,P < 0. 05). HBV DNA viral load positively correlated with expression levels of PD- 1 and PD- L1( r = 0. 393 and0. 271,respectively,all P < 0. 01),but showed no correlation with serum ALT,TBIL and PT levels. Conclusion The expression levels of PD- 1 and PD- L1 on CD4+T cells of CHB patients are closely related with disease progression,and they may be potential targets in treatment of CHB patients.
