隐匿性乙型肝炎血清学特点和高危感染因素调查

【摘要】 目的对隐匿性乙型肝炎流行现状进行调查,分析HBV隐匿性感染血清学特点和高危感染因素。方法对1023例2007年1月-2013年1月深圳市人民医院乙肝表面抗原阴性的样本进行乙肝DNA定量检测;采用1:1配对的病例-对照展开隐匿性乙型肝炎感染的相关流行病学调查,通过多因素条件logistic回归分析探讨隐匿性乙型肝炎的感染高危因素。结果核酸检测结果显示:1023例HBsAg(-)样本中检测出65例HBV隐匿性感染病例,携带率为6.4%。血清学分析显示HBeAg(+)+HBcAb(+)的血清学模式感染隐匿性肝炎的风险较高。多因素条件logistic回归分析显示:年龄(OR=2.6)、职业(OR=1.7)、经济条件(OR=3.0)和合并感染(OR=6.8)是隐匿性乙型肝炎的感染高危因素。结论隐匿性乙型肝炎在人群中呈低流行趋势,HBcAb(+)+HBeAg(+)是隐匿性乙型肝炎感染的高危血清型;年龄、职业、经济条件、合并感染。是隐匿性乙型肝炎感染的高危因素。

【Abstract】 Objective To investigate the prevalence of occult hepatitis B infection in Shenzhen,and analyze the serological characteristics and high risk factors of occult HBV infection. Methods From January 2007 to January 2013,1023 HBsAg negative patients were collected from Shenzhen people’s hospital for DNA quantitative detection of hepatitis B. Paired case-control was used to investigate the epidemiological characteristics of occult hepatitis B infection. The high-risk infection factors of occult hepatitis B infection were analyzed by multiple conditional logistic regression analysis. Results The DNA detection results showed 65 occult hepatitis B infection cases were detected from the 1023 HBsAg(-) samples,the carring rate was 6. 4%. Serological analysis showed that the model of HBeAg( +) + HBcAb( +) was high risk for occult hepatitis B infection. Logistic regression analysis showed the susceptible factors of occult hepatitis B infection were age( OR = 2. 6),occupation( OR = 1. 7), economy status( OR = 3. 0) and multiple infections( OR = 6. 8). Conclusion Occult hepatitis B infection showed low prevalence in population. Age,occupation,economic conditions,multiple infections are the high risk factors of occult hepatitis B infection.
