兔髌韧带重建术后骨-腱愈合磁共振定量评价的初步研究

【摘要】 【目的】探讨兔髌韧带重建术后骨-腱愈合的磁共振T2-mapping定量评估价值。【方法】新西兰兔34只(18周龄),随机分为对照组(7只)和手术组(4、8、12周各9只共27只)。手术组进行左膝髌韧带骨-腱重建,对照组不手术,术后第4、8、12周两组均行磁共振T2-mapping扫描成像并矢状面测量骨-腱接点(A兴趣区)T2值,比较手术组和对照组A区T2值的组间及组内差异,并与组织学HE染色对照。【结果】手术组A区不同时间点T2值均较对照组增高,差异有统计学意义(P=0.000,0.000,0.000);手术组A区T2值随着时间延长而降低,差异有统计学意义(P=0.000)。组织学显示,随着时间推移骨-腱交界区胶原纤维含量增多。【结论】T2-mapping技术对髌韧带重建术后骨-腱组织愈合的定量评价具有可行性,随着时间的延长骨-腱接点T2值逐步降低,T2值变化与水分含量、骨-腱交界区胶原纤维的增加具有潜在相关性。

【Abstract】 【Objective】 To explore the application value of T2-mapping quantitative assessment the healing after bone-tendon reconstruction in rabbits. 【Methods】 34 New Zealand white rabbits(18 weeks old) were randomly divided into control group(7??)and surgical group(27), and the surgical group were randomly divided into three groups(4 weeks, 8 weeks, and 12 weeks), the each is nine. The surgical group underwent bone-tendon reconstruction in left knees, and the control group is not surgery. After 4, 8,and 12 weeks of healing, both of surgical and control groups were underwent T2-mapping scan on the sagittal plane, and the T2 values of bone-tendon(ROI=A) were measured. The differences compared between inter-group and intra-group of surgical and control group of A by statistical analysis, which were compared with HE staining. 【Results】 Means for T2 values at A of surgical group higher than control, and there was statistically significant difference(P = 0.000; 0.000; and 0.000). Means for T2 values at A of surgical inter-group were decreased following time, and the statistically significance was different(P = 0.000). The collagen fiber of bonetendon junction was increasing follow-up. 【Conclusions】 It was useful to quantitative evaluation for tissue healing of bone-tendon reconstruction on T2-mapping, and the T2 values decreased following time. It was potential relevance between moisture content,collagen fiber increased and T2 values decreased of bone-tendon junction.
