负压吸引在微创经皮肾镜钬激光碎石术中的应用研究

【摘要】 目的探讨在微创经皮肾镜取石术中负压吸引的应用及其可行性。方法 2010年10月-2013年10月,对肾结石及上段输尿管结石72例患者按床号随机分成2组,对照组(单号)33例采用微创经皮肾镜钬激光碎石术,实验组(双号)39例,采用获得国家专利的自制能连接负压吸引装置的碎石通道行微创经皮肾镜钬激光碎石术,术中连接负压吸引,记录术中肾盂压力、碎石取石时间、术后患者体温变化情况并进行分析。结果实验组肾盂压力、碎石取石时间(结石体表面积/开始碎石取石到取石结束时间)、术后平均体温及发热持续时间明显小于对照组(P<0.05)。两组患者的结石清除率、住院天数差异无统计学意义。结论术中肾盂适当低压能减少术后发热发生率及持续时间。自制负压吸引碎石通道能降低肾盂内压并提高碎石取石效率。

【Abstract】 Objectives To investigate the feasibility of negative pressure suction in minially invasive percutaneous nephrolithotomy surgery( MPCNL). Methods All 72 patients with urology kidney stones and upper ureteral calculi in our department between Oct. 2010 and Oct. 2013 were assigned randomly into control group and experimental group( n = 33 and n = 39). The control group was treated with traditional MPCNL. In experimental group,a home-made negative pressure suction system which has obtained national patent protection was used in MPCNL. The renal pelvis pressure,lithotripsy time and body temperature changes were recorded for each group. Results The data of renal pelvis pressure,lithotripsy time and body temperature changes in experimental group was significantly less than which of the control group( P < 0. 05). However,the stone-free rate and length of hospital stay showed no significantly difference between control group and experimental group. Conclusion The low pressure in renal pelvis can reduce the incidence of postoperative fever and the duration of postoperative fever. The home-made negative pressure suction system can be used to reduce the pressure in renal pelvis during MPCNL and improve efficiency of percutaneous nephrolithotomy.
