同型半胱氨酸与帕金森病患者认知、情绪、病情进展的关系
The relationship between homocysteine,malonialdehyde and the severity,progression,cognition,emotion of Parkinson’s disease patients
【摘要】 目的探讨帕金森病(PD)患者血浆同型半胱氨酸(Hcy)、叶酸、维生素B12、丙二醛(MDA)浓度与病情严重程度、病程进展及认知和情绪的关系。方法通过检测PD患者的Hcy、叶酸、维生素B12、丙二醛浓度,并应用简明智力量表(MMSE)测查PD患者的认知功能、汉密顿抑郁量表(HAMD)测查PD患者的情绪,并结合患者的临床特征进行分析。结果帕金森患者中Hcy升高占57%。Hcy升高组与Hcy正常组比较,叶酸和维生素B12低,MDA增高,H-Y分期评分高,MMSE低,HAMD增高。结论 Hcy增高的PD患者病情较严重,更易出现认知功能受损和抑郁情绪,叶酸、维生素B12缺乏是Hcy增高的营养代谢因素,Hcy增高可能诱导氧化应激而导致多巴胺能神经元损害。补充叶酸和维生素B12降低PD患者血浆Hcy水平,可能延缓PD疾病进展,改善预后。
【Abstract】 Objective To explore the relationship between the concentration of homocysteine( Hcy),folic acid,vitamin B12,malonialdehyde( MDA) and the levels of progression,cognition,emotion of Parkinson’s disease( PD) patients. Methods The concentrations of Hcy,folic acid,vitamin B12,MDA were detected and cognitive function was evaluated by Mini-mental State Examination( MMSE) and emotion was measured by Hamilton Depression scale( HAMD) in PD. The clinical characteristics of the patients were analyzed. Results The level of Hcy of 57% PD was increased. The levels of folic acid,vitamin B12 in Hcy increasing group were decreased,the level of MDA was increased,H-Y staging scores were higher,the score of MMSE was lower and the score of HAMD was higher compared with those of Hcy norma group. Conclusions The high Hcy level might enhance oxidative stress and induce the dopaminergic neuron damage. The supplements of folic acid and vitamin B12 are helpful to reduce the level of Hcy,delay progression and improve prognosis in PD patients.
