肺炎克雷伯菌铁载体对喹诺酮类抗生素耐药性的影响

【摘要】 为载体与喹诺酮类抗生素耐药性之间的关系,采用K-B纸片法及微孔稀释法确定细菌对抗生素的药物敏感性,使用CAS琼脂法和紫外可见分光光度法分别对肺炎克雷伯菌铁载体进行定性和定量检测,使用统计学软件SPSS分析细菌铁载体与其耐药性的关系。CAS琼脂法分析表明,临床分离的70株肺炎克雷伯菌均产铁载体(100%),紫外分光光度法分析表明肺炎克雷伯菌中铁载体产量不同。根据统计学取中位数的方法能够将细菌分为铁载体高产组和低产组两组,喹诺酮类耐药株中铁载体产量高于敏感株(P<0.01);铁载体高产组对环丙沙星(CIP)和左氧氟沙星(LEV)的耐药率高于低产组(P<0.01);肺炎克雷伯菌铁载体与细菌对喹诺酮类抗生素的耐药性呈正相关性(P<0.01)。研究表明细菌铁载体参与了细菌耐药,干预了抗生素的杀菌过程。

【Abstract】 To explore the relationship between siderophores and quinolone resistance in bacterial,K-B disk diffusion method and microporous dilution method were used in determination of antimicrobial susceptibility in the clinical isolates.CAS agar and UVvisible spectrophotometric assay were used for qualitative and quantifiability detections of siderophore.Association between production of siderophores and quinolone resistance were statistically analyzed by use of SPSS.According to CAS agar plate assay,70 clinical isolates of Klebsiella pneumoniae all produce siderophores,however the production of siderophores measured by using UV-visible spectrometric method were different.Isolates were grouped into high siderophore-yielding group and low siderophoreyielding group based on median number.Siderophores in resistant strains of quinolone were significantly higher than the sensitive strains(P<0.01).The rate of ciprofloxacin and levofloxacin resistance in Klebsiella pneumoniae from the high siderophoreyielding group were significantly higher than those in low siderophore-yielding group(P<0.01).Siderophore in Klebsiella pneumoniae was positively correlated with bacterial resistance to quinolones(P<0.01).The results show that siderophores involved in bacterial resistance and interfere the sterilization process of antibiotics.
