【摘要】 [目的]分析SAPHO综合征(滑膜炎、痤疮、脓疱疮、骨肥厚、骨炎综合征)临床及影像学表现。[方法]回顾SAPHO综合征26例病人的临床及影像学表现,并查阅相关文献。[结果]SAPHO综合征是一种累及皮肤、骨和关节的慢性疾病;本组26例,其骨和关节病变的表现为:病人前上胸壁疼痛,胸肋和胸锁关节活动受限,其次是累及胸腰部6例;骶髂关节受累者2例;颞骨和下颌骨病变各1例。皮肤病变14例,为掌跖脓疱疮病(palmoplantar pustulosis,PPP)和痤疮。且皮肤病变具有性别特点,即脓疱病以女性为主,严重的座疮,以男性多见。影像学表现为:骨骼硬化、增厚、骨炎和关节炎等改变。好发部位在前上胸壁、骨盆、脊柱等,其中最具有特点的是胸肋、胸骨角区的改变,表现为骨肥厚、骨炎,胸肋关节和胸锁关节活动受限。[结论]SAPHO综合征临床报道极少,除与本征罕见以外,重要的是与临床和影像科医师应对本征的认识不足,漏诊、误诊有关。故应提高认识,正确诊断,积极治疗。更多还原

【Abstract】 [Objective] This study aimed to summarize the clinical and image features of synovitis- acne- pustulosis- hyperostosis- osteomyelitis( SAPHO) syndrome. [Methods] The clinical and image data of 26 cases with SAPHO syndrome were retrospectively studied,and the related literatures were reviewed. [Results] SAPHO syndrome was a kind of chronic disorder that involves the skin,bone and joints. The bone and joint disorders of the studied 26 cases were up anterior chest wall pain,limitations of motion of chest rib and sternoclavicular joints,6 cases with chest waist involved,2 cases with sacroiliac joint involved,1 case with temporal bone lesion and 1 case with mandible lesion. There were 14 cases with skin lesions,i. e.,palmoplantar pustulosis( PPP) mainly in females and serious acne mainly in males. The image features were bone sclerosis,hyperostosis,osteitis,arthritis,etc. The lesions were mainly found in up anterior chest wall,pelvis,spine,etc. One of the most characteristic was the change in sternal rib and sternal angle region,which mainly exists as hyperostosis,osteitis,and limitations of motion of sternocostal joint and sternoclavicular joint. [Conclusion] SAPHO syndrome is rarely reported. Besides its low incidence,this is due to the insufficient understanding of this disease in clinical practice and the missed diagnosis and misdiagnosis.
