贵阳市部分事业单位乳腺癌发病情况的初步调查
The primary investigation on the incidence of breast cancer in the institution of Guiyang city
【摘要】 目的了解贵州省贵阳市部分事业单位在职及退休女职工乳腺癌发病情况,初步探讨适合贵阳市事业单位妇女乳腺癌筛查的适宜技术。方法选取2011年6月至2102年5月间贵阳市12089例事业单位在职和退休女职工进行乳腺癌筛查,筛查方法以专科体检(CBE)和高频彩超(HFCDU)为主,数字钼靶X线(FFDM)作为筛查方法的补充,以病理活检为金标准,计算并比较CBE、HFCDU和CBE联合HFCDU的灵敏度、特异度、正确诊断指数和诊断指数。结果筛查共发现乳腺癌10例,总检出率82/10万,10例都为早期乳腺癌(包括0期和Ⅰ期),发病年龄为34~70岁,高发年龄为40~60岁,中位年龄46.5岁。比较各年龄段乳腺癌检出率,差异有统计学意义(P<0.05)。统计学分析比较3种筛查方式的灵敏度,差异无统计学意义(P>0.05),3种筛查方法特异度差异有统计学意义(P<0.05),其中专科体检联合高频彩超的特异度、正确诊断指数和诊断指数最高。结论2011至2102年间贵州省贵阳市部分事业单位在职及退休女职工乳腺癌检出率82/10万,高发年龄为40~60岁。CBE联合HFCDU作为贵阳市事业单位妇女乳腺癌筛查手段是可行的,联合FFDM可提高诊断率。
【Abstract】 Objective To understand the incidence of breast cancer in Guiyang female employees,and explore appropriate technologies for breast cancer screening. Methods They carried out breast cancer screening for part of female employees about 12089 people in parts of Guiyang from June 2011 to May 2102,The Clinical breast examination( CBE),high-frequency ultrasound( HFCDU) is the primary means,the digital X-ray mammography( FFDM) as the screening method supplement,biopsy as the gold standard,to calculate and compare CBE,HFCDU、and CBE combined HFCDU sensitive,specificity,Youden’s inde and the diagnostic index. Results The 10 cases of breast cancer were found in the breast cancer screening,the total detection rate of 82 /100000,early breast cancer( including 0,Ⅰ period) in 10 cases,age 34 to70 years old,high age of 40 to 60 years old,with a median age of 46. 5 years. The incidence of different ages was significantly different( P < 0. 05). The sensitive was not statistically significant( P > 0. 05). The specificity difference was statistically significant( P < 0. 05). Conclusion From 2010 to 2012,the detection rate was 82 /100 0000 in part of female employees of Guiyang city,40 to 60 years old was the peak age.Using Clinical breast examination combined high frequency ultrasound as a screening tool for breast cancer screening in the Guiyang. Meanwhile the united FFDM could improve the diagnosis rate.
