【摘要】 目的 总结在下肢深静脉血栓(DVT)治疗中应用个体化治疗方案的临床经验。方法 对2007年9月至2012年3月在安徽省池州市人民医院及江苏南通大学附属医院住院的80例下肢DVT患者,分别针对不同病情给予个体化治疗:20例周围型DVT、下肢症状不严重患者应用单纯抗凝治疗,26例亚急性、无肺栓塞但有肢体肿胀组患者应用下肢浅静脉阻断深静脉灌注溶栓治疗,10例下肢混合型血栓、发病时间小于1周的患者采用深静脉置管溶栓治疗,24例下肢中央型血栓、发病时间小于1周的患者采用腔静脉滤器置入治疗,对临床效果进行观察。结果 4组患者经个体化治疗后,治疗显效率为76.3%(61/80),有效率为16.3%(13/80),并发症的发生率为7.5%(6/80)。其中2例合并肺栓塞患者,治疗效果良好,有3例存在抗凝禁忌,有1例因脑出血死亡;治疗后,4组患者健、患肢膝关节上、下15 cm处周径差均比治疗前明显下降,差异有统计学意义(P<0.05)。结论 对于不同病情的下肢DVT患者实施个体化治疗方案疗效确切,治疗效果好。更多还原

【Abstract】 Objective To summarize the clinical experience of customized therapy in deep venous thrombosis.Methods Totally 80 patients underwent customized therapy from September 2007 to March 2012.20 cases with peripheral deep venous thrombosis and lower extremity symptoms were treated with puring anticoagulation.26cases with of subacute but no pulmonary embolism in limb swelling were treated with deep venous perfusion thrombolysis with superficial venous blocked.10 cases with mixed lower limb thrombosisless than one week were treated with catheter directed thrombolysis.24 cases of with central lower limb thrombosis less than one week were treated with inferior vena cava filters placement.Results After individualized treatment,the significant effective rate was76.3%(61/80);effective rate was 16.2%(13/80);the incidence of complications was 7.5%(6/80).Among them,the therapeutic effect of 7 pulmonary embohsm was good.There were 3 cases of anticoagulant taboo and 3cases died of cerebral hemorrhage.After treatment,the perimeter difference of health and limb knee joint in four groups were significantly decreased(P<0.05).Conclusion Customized therapy in deep venous thrombosis is reliable.更多还原
