【摘要】 目的比较替米沙坦和氨氯地平对冠心病合并轻度高血压患者颈动脉内膜中层厚度(IMT)及炎症因子的影响。方法选取冠心病合并轻度高血压患者150例随机分为两组,氨氯地平组(n=75,口服氨氯地平5 mg+阿托伐他汀钙20 mg,1次/d),替米沙坦组(n=75,口服替米沙坦80 mg+阿托伐他汀钙20 mg,1次/d),共治疗1年,比较治疗前后两组患者血压、IMT、hs-CRP及MMP-9水平。结果两组治疗后较治疗前收缩压及舒张压均降低,差异有统计学意义(P<0.05)。替米沙坦组治疗12个月的IMT小于基础值(P<0.05)。替米沙坦组治疗12个月的hs-CRP及MMP-9均低于治疗前及氨氯地平组,差异均有统计学意义(P<0.05)。结论替米沙坦应用于冠心病合并轻度高血压患者的抑制炎症反应作用优于氨氯地平,其在降压达标基础上抑制炎症反应,延缓颈动脉内膜增厚进程,其临床获益更大。更多还原

【Abstract】 Objective To compare influence of telmisartan and amlodipine on carotid intima-media thickness and inflammatory factors in patients with coronary heart disease combined with mild hypertension. Methods 150 patients with coronary heart disease combined with mild hypertension were selected and randomly assigned to two groups.Amlodipine5 mg and atorvastatin 20 mg,once a day were used in amlodipine group(n=75),telmisartan 80 mg and atorvastatin 20 mg,once a day were applied in telmisartan group(n=75).Treatment for 1 year,blood pressure,IMT,hs-CRP and MMP-9 level in two groups before and after treatment were compared. Results The systolic pressure and diastolic pressure after treatment in two groups was lower than that before treatment respectively,with statistical difference(P<0.05).IMT after treatment of 12 months in telmisartan group was little than that the basic value(P<0.05).hs-CRP and MMP-9 level treatment of 12 months in telmisartan group was lower than that before treatment and amlodipine group respectively,with statistical difference(P<0.05). Conclusion Telmisartan applied in patients with coronary heart disease combined with mild hypertension has good inhibition of inflammatory reaction than amlodipine,it decreased the inflammation response on the basis of control blood pressure and delay IMT process,has greater clinical benefit.更多还原
