【摘要】 目的比较CEUS与CECT对不同病理分期及分级CCRCC的诊断价值。方法分析83例经病理证实为CCRCC(共87个)的CEUS及CECT诊断结果,与TNM病理分期、Fuhrman病理分级结果进行比较,统计并进行一致性检验。结果 CEUS与CECT对不同TNM病理分期及Fuhrman分级CCRCC的诊断效能均无统计学差异(P>0.05),具有中度一致性(Kappa=0.577,P<0.000 1)。TNM病理分期及分级不同,两者的诊断效能一致性不同,T1a期一致性最低(Kappa=0.290,P=0.039),T1b期及Ⅲ级一致性最高(Kappa=1,P<0.0001)。结论不同TNM病理分期及Fuhrman分级CCRCC,CEUS与CECT的诊断效能一致性不同。CEUS与CECT联合应用,能为CCRCC的诊断,尤其是低分期及低分级CCRCC的诊断,提供更多依据。更多还原

【Abstract】 Objective To compared and evaluate the diagnostic value of contrast-enhanced ultrasound(CEUS) and contrast-enhanced CT(CECT) in diagnosis of CCRCC in different TNM stage and Fuhrman grade.Methods CEUS and CECT test results for Eight-three CCRCC patients who were pathologically confirmed within 87 tumors were analyzed with the results of TNM stage and Fuhrman grade respectively.Results There were no significant difference between CEUS and CECT(P>0.05) in each TNM stage and Fuhrman grade of CCRCC,and consistence was medium(Kappa=0.577,P<0.000 1).Different consistence in CEUS and CECT of diagnosis of CCRCC was found in different TNM stage and Fuhrman Grade.High consistency was found between CECT and CEUS in diagnosis of CCRCC both in the Tlb stage and the Ⅲ grade(Kappa=1,P<0.000 1) but low in Tla(Kappa=0.290,P=0.039).Conclusions Consistencies were different in different TNM stage and Fuhrman grade.the combination of CEUS and CECT would improve the diagnostic information for CCRCC,especially in lower TNM stage and Fuhrman grade.更多还原
